紫杉醇联合卡铂治疗116例晚期初治卵巢上皮癌近期严重不良反应临床观察

摘　　要：目的观察紫杉醇联合卡铂化疗方案（paclitaxelandcarboplatin，TC方案）治疗晚期初治卵巢上皮癌不同疗程期间出现的近期严重不良反应情况，为临床诊治提供一定的参考。方法回顾性分析TC3周方案治疗116例晚期初治卵巢上皮癌患者6～8个疗程期间所发生的近期严重不良反应，不良反应采用抗癌药物毒性反应分级（WHO）进行评定。结果在6个疗程及8个疗程后分别有27.6%、36.6%的患者先后出现了严重（Ⅲ～Ⅳ度）的近期不良反应，主要表现在血液系统方面，尤其至6个疗程后，与4个疗程后比较，差异具有统计学意义（P=0.003）；且继续化疗，血液学系统严重不良反应发生率仍有上升趋势。结论多疗程（尤其在6个疗程后）TC方案治疗晚期初治卵巢上皮癌期间应注意患者血液系统可能出现严重的不良反应，临床上应引起重视。Objective To observe and compare the recent severe adverse reactions ofpaclitaxel and carboplatin chemotherapy in the initial treatment of advanced epithelial ovariancancer during different cycles of treatment so as to provide a reference for clinical diagnosis andtreatment.Methods Recent severe adverse reactions that occurred in 116 patients with advancedepithelial ovarian cancer treated with TC chemotherapy for 6 to 8 cycles of triangle -weekchemotherapy were retrospectively reviewed and recorded. Adverse reactions were evaluatedand compared with WHO grade .The data were analyzed through SPSS16.0 statistical softwaresystem.Results Adverse reactions( Ⅲ ~ Ⅳ degree) appeared mainly in the hematological systemof 27.6% of cases after 6 cycles and 36.6% of cases after 8 cycles among 116 patients. Thedifference between cases after 6 and 4 cycles of treatment was statistically significant(P=0.003).The incidence of severe adverse reactions in hematological system is still rising with continuedchemotherapy.Conclusions Attention should be paid to the adverse reactions of hematologicalsystem during the initial treatment of advanced epithelial ovarian cancer with TC chemotherapy,especially after 6 cycles of treatment.
