摘  要:目的探讨CK7、CK19在肝细胞癌(HCC)中的表达,以及与HCC患者甲胎蛋白(AFP)表达及临床病理特征的关系。方法收集65例原发性HCC患者的癌组织标本及血清标本,免疫组化检测CK7、CK19表达情况,并用ELISA法检测血清中AFP水平。结果 HCC患者癌组织中CK7的表达阳性率为46.2%(30/65),CK19表达阳性率为18.5%(12/65)。HCC癌组织中可见表达CK7、CK19的卵圆细胞,可同时表达AFP。CK19表达与血清AFP表达水平有显著相关性(P=0.000)。结论人类HCC中同样有卵圆细胞的存在,这部分HCC患者预后较差,AFP及CK19的联合检测可有助于这部分患者的早期诊断。Objective To investigate the expression of CK7 and CK19 in hepatocellular carcinoma (HCC) and its relation to clinicopathologic features. Methods Serum and tissue samples were collected from 65 patients with HCC. The expression of CK7 and CK19 in HCC tissue was detected with immunohistochemistry, the serumAFP levels were measured with enzyme-linked immunoassay (ELISA) method. Results The positive expression rates of CK7 and CK19 in HCC tissue were 46.2% (30/65) and 18.5%(12/65), respectively. Oval cells were detected in HCC which expressed CK7, CK19 and AFP. The expression of CK19 in HCC was significantly correlated with serum AFP levels (P=0.000). Conclusion The bipotent hepatic progenitor cells (HPCs) in HCC express CK19 and AFP, indicating that combined detection of tissue CK19 expression and serum AFP level may be helpful in early diagnosis of hepatocellular carcinoma with HPCs.
