摘  要:目的:探讨恶性肿瘤中主动呼吸控制状态下CT与自由呼吸状态下四维CT的应用。方法:收治行立体定向放射治疗的肺部恶性肿瘤患者17例,分别在主动呼吸控制状态下行常规CT扫描(ABC-CT)、自由呼吸状态下行常规CT扫描(FB-CT)与四维CT扫描(4D-CT),比较检查效果。结果:4D-CT的定位时间、治疗时间低于ABC-CT(P<0.05);4D-CT的PGTV/GTV、PGTV、静态GTV高于ABC-CT(P<0.05);与FB、MeanIP-CT、CTO比较,ABC的双肺体积明显增大,把区定位更为精确,正常肺组织照射剂量明显减少。结论:ABC-CT与4D-CT在放射治疗中均能获得比较理想的定向效果,但ABC方式的临床特征更为理想。Objective:To explore the application of four dimensional CT under free breathing and CT under active breathingcontrol state in malignant tumor.Methods:17patients with lung cancer treated with stereotactic radiotherapy were selected.Theywere given ABC-CT,FB-CT and4D-CT respectively.We compared the examination results.Results:The location time andtreatment time of4D-CT was lower than ABC-CT(P<0.05).PGTV/GTV,PGTV,static GTV of4D-CT were higher thanABC-CT(P<0.05).Compared with FB,MeanIP-CT and CT0,in ABC,the double lung volume was obviously increased,the targetarea positioning was more accurate,and the radiation dose of normal lung tissue was obviously decreased.Conclusion:In radiationtherapy,both ABC-CT and4D-CT can achieve more ideal directional effect,but the clinical features of ABC were more ideal.
