姑息性手术对老年胃癌患者的临床疗效及预后分析

摘　　要：目的观察老年晚期胃癌患者接受姑息性手术治疗的疗效,以期为老年胃癌患者临床合理治疗提供依据。方法选择老年胃癌患者106例,随机分为试验组和对照组,各53例,对照组行根治术,试验组行姑息性手术治疗。观察并比较两组患者治疗期间手术时间、住院天数、术后输血量,术后并发症,1年、3年、5年生存率。结果试验组和对照组患者在治疗期间手术时间[(257.8±61.3)d vs(263.5±63.8)d]、平均住院天数[(16.7±3.8)d vs(17.4±4.1)d]、术后并发症(92.5%vs 86.8%)、术后1年的生存率(92.5%vs 86.8%)比较差异无统计学意义,而对照组术后输血量多于试验组[(372.1±89.6)m L vs(325.3±90.4)m L](P<0.05);对照组术后3年(84.9%vs 56.6%)、5年(62.3%vs 28.3%)的生存率均优于试验组,差异有统计学意义(P<0.05)。结论与根治术比较,姑息性手术对老年胃癌患者创伤小,安全性高。Objective To observe the curative effect of palliative operation for elderly patients with advanced gastric cancer in order to providethe basis for clinical treatment of elderly patients with gastric cancer.Methods106cases of elderly patients with gastric cancer were divided intothe experiment group and control group,53cases in each group.The control group were given radical resection,the experimental group were givenpalliative surgery.Operation time,length of stay,postoperative blood transfusion,postoperative complications,1,3,5years survival rates were observedand compared.Results There were no significant differences in operation time[(257.8±61.3)d vs(263.5±63.8)d],length of stay[(16.7±3.8)d vs(17.4±4.1)d],postoperative complications(13.2%vs15.1%),1-year survival rate(92.5%vs86.8%)between the experimental group and the controlgroup.The postoperative blood transfusion of control group was more than experimental group[(372.1±89.6)mL vs(325.3±90.4)mL](P<0.05).3-year survival rate of group(84.9%vs56.6%),5-years(62.3%vs28.3%)survival rate of control group the was better than experiment group(P<0.05).Conclusion Compared with radical mastectomy,palliative surgery for elderly patients with gastric cancer is minimally invasive and safe.
