整形保乳术与常规保乳术治疗早期乳腺癌比较

摘　　要：目的:比较整形保乳术与常规保乳术治疗早期乳腺癌的疗效与安全性,指导临床治疗策略的选择。方法:2010年1月—2012年1月145例病理分期Ⅰ~Ⅱ期早期乳腺癌患者,整形保乳组74例,常规保乳组71例,调取两组患者一般资料、手术情况、术后并发症、预后情况,并评价两组患者术后5年生活质量差异。结果:整形保乳组手术时间、乳房美观度优良率、SF-36量表各维度得分及平均得分高于常规保乳组,其术后引流量及住院时间,术后血肿、皮下出血、切口愈合不良发生率低于后者,差异有统计学意义(P<0.05)。两组患者5年生存率、局部复发率、远处转移率比较,差异无统计学意义(P>0.05)。结论:整形保乳术与常规保乳术治疗早期乳腺癌均可取得确切的治疗效果,整形保乳术具有更为理想的美容效果,对患者早期恢复与生活质量的提高具有积极意义,较常规保乳术存在明显优势。Objective:The objective of this study was to explore the correlation between CG(cystitis glandularis,CG)and BC(bladder cancer,BC)and investigate the canceration rate of CG.Methods:The patients diagnosed with CG for the first time and with complete readmission information from Jan2006to Dec2016were given tracing analysis and the canceration rate of CG was analyzed.The patients with CG were classified as non-canceration group and the patients who have already worsened into BC were classified as canceration group.The difference of age,gender,course of disease,toxic exposure and complication between two groups were compared and the factors with statistical difference were incorporated in Logistic multi-factors analysis and the factors affecting the CG into BC were summarized.Results:A total of173cases were selected in this study,of whom21cases were diagnosed with BC and the CG canceration rate was12.14%.multi-factor analysis showed that the course of disease for more than1year,toxic exposure for over1year,intestinal epithelial type and bladder stones were the independent risky factors effecting CG canceration and the risk degree of these factors were over10(P<0.05).Conclusions:The CG canceration rate is rather high,which is probably the middle segment that normal BC developed into.The patients with long term course of disease,toxic exposure,intestinal epithelial type and bladder stones are more likely to get BC,who should be given intensive treatment,close follow-up to detect the canceration in advance.
