摘 要:目的比较飞秒激光和机械板层刀制瓣准分子激光原位角膜磨镶术(laser in situ keratomileusis,LASIK)矫正高度近视术后角膜高阶像差的变化。方法采用回顾性分析的方法,选取2015-03至2016-03在解放军某医院眼视光中心分别接受飞秒激光和机械板层刀制瓣LASIK矫正高度近视的患者103例(200眼);其中接受飞秒激光制瓣记为A组(n=100);接受机械板层刀制瓣记为B组(n=100)。应用Nidek OPD SCAN波前像差分析仪在暗室环境下分别测量术前,术后10 d、1个月、3个月、6个月时的总高阶像差、球差、彗差、三叶草像差。结果术后A、B两组像差都有所增加,增幅:(1)两组内术后6个月与术前比较,差异具有统计学意义(P<0.05);(2)两组间总高阶像差、彗差及球差在术后10 d及1个月时差异有统计学意义(P<0.05);(3)两组三叶草差在术后第10天增幅差异有统计学意义(P<0.05)。结论飞秒激光和机械角膜板层刀制瓣LASIK术后像差均有所增加,飞秒激光原位角膜磨镶术在矫正高度近视方面对减少术后角膜高阶像差更具优势,利于提高术后视觉质量,可能是一种比较理想的手术方式。Objective This study objective was to compare the changes of high-order aberrations after laser in situ keratomileusis(LASIK)with femtosecond laser and mechanical microkeratome for flap cutting for correction of high myopia.Methods A retrospectiveanalysis method was used for this study,a total study sample of103patients(200eyes)that presented to the ophthalmology centre of a liberationarmy hospital between March2015and March2016were analysed.The sample was divided into two equal groups(group A and group B),51high myopia patients(100eyes)of group A were treated with femtosecond laser for flap cutting,and52high myopia patients(100eyes)of thegroup B were treated with mechanical microkeratome for flap cutting.The wave-front aberration analyser Nidek OPD SCan was applied tomeasure the total higher-order,spherical,coma,and wave-front aberrations in a dark room before the surgery,at10days,1month,3monthsand6months after surgery.Results Postoperative aberrations in group A and group B were both increased.Rate of increase:(1)as comparedwith those before surgery,the intra-group difference in aberrations six months after surgery in both two groups was statistically significant(P<0.05);(2)The difference in the total higher-order,coma and spherical aberrations10days and1month after surgery between the two groupswas statistically significant(P<0.05).(3)There was a statistically significant difference between the changes of wave-front aberration of thetwo groups10days after the surgery(P<0.05).Conclusions The aberrations after femtosecond laser and mechanical microkeratome forflap cutting in LASIK were both increased.Femtosecond LASIK is more suitable for the correction of high myopia,is helpful to improve thepostoperative visual quality,and may be an ideal surgical method.
