宫颈液基细胞学上皮病变与宫颈活检的临床病理分析

摘　　要：目的探讨宫颈液基薄层细胞学检测阳性结果与组织活检结果的符合情况,分析其临床应用价值。方法选取213例细胞学筛查阳性并进一步做组织活检的患者的临床病理资料进行回顾性分析,并分年龄组对比细胞学与活检结果的符合情况。结果液基细胞学阳性结果中ASC-US有85例,ASC-H有19例,LSIL有64例,HSIL有45例。相应宫颈活检结果中慢性宫颈炎有41例,低级别上皮内瘤变有86例,高级别上皮内瘤变有82例,浸润癌有4例。两者检测宫颈上皮内病变高发年龄为30~49岁,浸润癌发生在50~59岁。结论液基细胞学检测与宫颈活检有较高的符合率;阴道镜下多点活检有利于早期发现宫颈上皮内瘤变;细胞学发现宫颈上皮内病变者需进一步活检确诊。Objective To investigate the coincidence between the positive findings of the cervical thinprep cytology test and the cervical biopsy,and analysis the value of thinprep cytology test clinically.Methods Retrospective analysis the pathology information of213women,who had been take a biopsy because the thinprep cytology test is positive recently.Meanwhile,contrastive analysis the coincidence of the cytology and biopsy.Results The positive of the thinprep cytology test showed:ASC-US of85cases,ASC-H of19cases,LSIL of64cases,HSIL of45cases.The cervical biopsy showed:chronic cervicitis of41cases,Low grade intraepithelial neoplasia of86cases,High grade intraepithelial neoplasia of82cases,The infiltrating carcinoma of4cases.The cervical intraepithelial neoplasia has a high incidence rate in30-49ages.The infiltrating carcinoma was detectedin50-59ages.Conclusion There is high accordance rate of the thinprep cytology test and the cervical biopsy.The colposcopic multiple biopsies is helpful to find the intraepithelial neoplasia in early stage.The patient of cytology positive should be diagnosed by biopsy.
