摘　　要：目的:评估安氏Ⅱ类1分类成人患者拔牙矫治前后唇形态的变化。方法:本研究包括30例拔牙矫治的安氏Ⅱ类1分类成人患者,矫治前后均拍摄头颅侧位片,对20个软硬组织项目进行测量,并采用t检验比较矫治前后测量值,分析唇部形态的变化。结果:上唇-审美平面距(UL-EP)、下唇-审美平面距(LL-EP)、上唇突度(ULP)、下唇突度(LLP)、上下唇突度差(ULP-LLP)、上唇倾角(A′-UL-FH)、下唇倾角(B′-LL-FH)、鼻唇角(NLA)、Z角、上唇位置、上中切牙位置(U1-Ptm)、颏唇沟角(LL-B′-Pos)矫治前后的变化具有统计学意义(P<0.05);上下唇长度(ULL、LLL)、上唇厚度(AA′)、上下唇基角(S-Ns-Sn、S-Ns-B′)矫治前后的变化未见明显统计学差异(P>0.05)。各测量指标在不同性别间未见明显统计学差异(P>0.05)。结论:随着上切牙的后移,关于唇形态的各项美学指标(如:UL-EP、LL-EP等)均有显著改变,使得唇部形态及面下1/3侧貌均得到良好的改善,有利于面部整体协调美观的提升。Objective To evaluate the changes in the lip morphology before and after orthodontic extraction treatment in Class II division1malocclusion in adult patients.Methods A total of30Class II division1malocclusion adult patients with tooth xtraction orthodontic treatment were included in this study.Pre-and post-treatment cephalometric X-rays were performed,and20measurement items were measured.The change values of pre-and post-treatmentwere compared by t-test to analyze the changes of the lip morphology.Results There were statistically significant differences in UL-EP,LL-EP,ULP,LLP,A′-ULFH,B′-LL-FH,U1-Ptm,Z angle,LL-B′-Po,ULP-LLP,upper lip position and NLA(P<0.05).There were no statistically significantdifferences inULL,LLL,A-A′,S-Ns-Sn,S-Ns-B′(P>0.05).No statistically significant difference was found between male and female about each measurement index in this study(P>0.05).Conclusion With the retrusion of the upper incisors,the aesthetic standards of upper and lower lips have significantly changed so that the lip morphology and the lower1/3facial profile havesignificantly change.The improvements of the lip morphology and the lower1/3facial profile have great advantages to the harmony and aesthetics of the whole facial morphology.
