摘 要:目的:通过测定钛合金与镍铬合金烤瓷冠患者龈沟液内AST、ALP活性及内毒素的含量并研究其相关意义,来正确评估在口腔内使用普通烤瓷冠的安全性。方法:选取笔者科室做了全冠修复的120例患者作为研究对象,按照修复材料的不同分为两组:钛合金组60例,镍铬合金组60例。观察和记录患者龈沟液内AST、ALP活性水平及内毒素的含量并进行比较。结果:钛合金组和镍铬合金组戴临时烤瓷冠1周后AST值、ALP值以及内毒素的含量均突然上升,原因可能与树脂成形制作临时冠的方法有一定的联系。而AST值、ALP值以及内毒素的含量在戴牙1个月、3个月、6个月时均有所下降并逐渐趋于平缓,随着时间的变化,同一种金属的酶活性变化不具有统计学意义。ALP、AST活性值以及内毒素的含量略微高于牙体预备前,而对钛合金组和镍铬合金组进行不同时段的ALP、AST活性值以及内毒素的含量比较发现,钛合金组均比镍铬合金组低,差异具有统计学意义。结论:钛合金与镍铬合金烤瓷冠能影响牙周龈沟液内的内毒素以及酶活性,但随时间延长并未使牙周发生病变,因此,从材料学方面来看,临床上口腔内应用普通金属烤瓷冠的措施是可行的。Objective To evaluate the safety of using porcelain fused to metal crown in oral cavity through assessing the AST and ALP in gingival crevicular fluid of titanium alloy and Ni Cr alloy porcelain fused to metal crown.Methods All120patients who had been treated with crown restoration in our hospital were selected as the subjects of this study.The subjects were divided into two groups according to the different repair materials:Titanium alloy group60cases and Ni Cr alloy group60cases.The levels of AST,ALP activity and endotoxin in gingival crevicular fluid were observed and recorded.Results The value of AST and ALP and endotoxin content were increased suddenly after1week in titanium alloy and Ni Cr alloy porcelain crown temporary crown.The reasons may be related to the method of making temporary crown.The reasons may be relatedto the method of making temporary crown.The AST,ALP and the content of endotoxin were decreased at1,3,6months.With the change of time,the enzyme activity of the same metal was not statistically significant.The ALP,AST and the content of endotoxin activity slightly higher than before tooth preparation,The activity of ALP,AST and endotoxin in the titanium alloy group were lower than that of the Ni Cr alloy group,and the difference was statistically significant.Conclusion The effect of titanium alloy and Ni Cr alloy porcelain crown periodontal endotoxin in gingival crevicular fluid and enzyme activity,but time did not make periodontal disease,so it is feasible to apply the common metal porcelain crown in clinic from the aspect of material science.
