双颌垫矫治器治疗安氏Ⅱ类错[牙合]患者的牙性效果分析

摘　　要：目的:研究双颌垫功能矫治器矫治安氏Ⅱ类错畸形患者上下颌牙的变化。方法:选择11例安氏Ⅱ类错患者,双颌垫矫治器治疗平均约10个月,治疗结束后测量治疗前后头颅侧位片并对测量结果进行统计学分析。结果:所有病例经过平均10个月的功能矫治器治疗后,磨牙达到中性关系,前牙明显内收,上颌磨牙平均远中移动(U6-PTV)0.76mm,伸长(U6-PP)0.62mm;下磨牙平均近中移动(L6-PTV)0.90mm,伸长(L6-MP)0.61mm,差异都有统计学意义(P<0.05);上中切牙平均远中移动(U1-PTV)1.18mm,伸长(UI-PP)0.66 mm,下切牙平均近中移动(LI-PTV)1.06mm,差异有统计学意义(P<0.05),下切牙伸长(LI-MP)0.58mm,但差异无统计学意义(P>0.05)。结论:双颌垫功能矫治器对替牙或恒牙初期安氏Ⅱ类错患者有明显的治疗效果,它可以减少切牙的突度,减少覆盖,较好地改善Ⅱ类磨牙关系,提高安氏Ⅱ类错患者的治疗效果。Objective To evaluate the treatment effects of the Twin-block appliance on maxillary and mandibular teeth change of the patients with ClassⅡmalocclusion.Methods11patients with ClassⅡmalocclusion were selected for the study,During the treatment Twin-block appliance was used and the average period of treatment was around10months,meanwhile the cephalometric data were collected and statistical analysis were applied.Results After approximately10months of functional appliance therapy conducted with all patients,their molar relation acquired neutral state,front teeth got adduction and normal covering state in obvious manner.The upper molar moved distally(U6-PTV)0.76mm,elongated(U6-PP)0.62mm on average,mandibular molar moved mesially(L6-PTV)0.90mm,elongated(L6-MP)0.61mm on average,all these differences were statistically significant(P<0.05);the average distal movement of upper central incisor(U1-PTV)was1.18mm,elongation was(UI-PP)0.66mm,lower incisors moved mesially(LI-PTV)1.06mm on average,these differences were all statisticallysignificant(P<0.05),but its elongation(LI-MP)was0.58mm,which was not statistically significant(P>0.05).Conclusion Twin-block appliance is more effective when treating patients with classⅡmalocclusion at mixed and early permanent dentition.It could reduce protrusion of the upper incisor and reduce overjet,and by improving the classⅡmolar relationship increase its treatment effects on classⅡmalocclusion.
