TMR对直肠癌诊断、分期及术前评估的临床应用价值分析

摘　　要：目的分析TMR对直肠癌诊断、分期及术前评估的临床应用价值。方法 40例术前经过结肠镜活检确诊的直肠癌患者盆腔进行磁共振的扫描,并对直肠癌进行影像的分期以及术前的评估,仔细观察患者病变发生的部位,探讨表观扩散系数(ADC)值和直肠癌恶性程度的相关性,分析3.0T磁共振的常规数列和动态对于增强直肠镜诊断直肠癌的优势。结果当扩张散加权系数(b)=1200 s/mm^2时,高分化直肠癌ADC值为(1.17±0.13)、中分化直肠癌ADC值为(0.96±0.12)、低分化直肠癌ADC值为(0.87±0.12),三组比较差异均有统计学意义(P<0.05)。磁共振成像(MRI)矢状位置能够准确的测量出肿瘤下缘到肝缘之间的曲线距离。结论在对直肠癌诊断、分期及术前评估的临床应用价值分析的过程中,3.0T MR在临床上能够对直肠癌进行科学准确诊断、分期以及术前的评估,从而为临床医生手术方案的科学制定提供重要的依据,非常值得在临床上进行大力的推广和应用。Objective To analyze the clinical application value of TMR for diagnosis,staging and preoperative evaluation of colorectal cancer.Methods A total of40colorectal cancer confirmed by colonoscopy biopsy magnetic resonance imaging scanning on pelvic cavity,evaluation were made on image staging and preoperative assessment of colorectal cancer.Lesions occurred parts in patients were carefully observed to explore correlation between apparent diffusion coefficient(ADC)values and colorectal malignant degree,and analyze the advantages of3.0T magnetic conventional sequence and the dynamic state in enhancing diagnosis of rectoscope for colorectal cancer.Results When diffusion sensitive gardient(b)=1200s/mm2,high differentiation of colorectal cancer had ADC value as(1.17±0.13),middle differentiation of colorectal cancer had ADC value as(0.96±0.12)and low differentiation of colorectal cancer had ADC value as(0.87±0.12).Their difference had statistical significance(P<0.05).Magnetic resonance imaging(MRI)sagittal position can accurately measure the curve distance between edge of tumor and liver edge.Conclusion In analyzing clinical application value of colorectal cancer diagnosis,staging and preoperative assessment process,3.0T MR can make scientific and accurate diagnosis,staging and preoperative assessment,so as to provide important basis for scientific formulation of surgical project for clinicians.So it is worth vigorous promotion and application in clinic.
