血小板相关指标在短暂性脑缺血发作患者中的检测价值研究

摘　　要：目的研究分析血小板相关指标在短暂性脑缺血发作患者中的检测价值。方法 66例短暂性脑缺血发作患者设为观察组,66例体检健康者设为对照组,检测比较两组研究对象及观察组中不同ABCD2评分者血小板参数及血小板活化指标。结果 (1)血小板参数:对照组的血小板计数(PLT)、平均血小板体积(MPV)和大血小板比例(P-LCR)分别为(140.15±13.45)×10~9/L、(11.21±1.32)fl及(36.75±3.31)%,观察组的PLT、MPV及P-LCR分别为(118.98±10.24)×10~9/L、(14.20±1.56)fl及(42.20±5.17)%,观察组的PLT低于对照组,其他血小板参数均高于对照组,差异均具有统计学意义(P<0.05)。观察组中ABCD2评分较高者PLT低于评分较低者,其他血小板参数均高于评分较低者,差异均具有统计学意义(P<0.05)。(2)血小板活化指标:对照组的CD61、α颗粒膜蛋白-140(GMP-140)及血小板膜糖蛋白Ⅱb/Ⅲa(GPⅡb/Ⅲa)分别为(38.38±3.52)%、(8.07±0.72)%及(16.26±2.53)%,观察组的CD61、GMP-140及GPⅡb/Ⅲa分别为(46.35±4.23)%、(13.34±1.35)%及(24.95±3.37)%。观察组的各项血小板活化指标均高于对照组,差异均具有统计学意义(P<0.05)。观察组中ABCD2评分较高者各项血小板活化指标均高于评分较低者,差异均具有统计学意义(P<0.05)。结论血小板相关指标在短暂性脑缺血发作患者中的检测价值较高,且对本类患者的预后与诊治均有一定的指导意义。Objective To research and analyze detection value by platelet-related parameters in transientischemic attack patients.Methods There were66patients with transient ischemic attack as observation groupand66healthy people in physical examination as control group.Detection and comparison were made on plateletparameters and platelet activation indexes between the two groups and in patients with difference ABCD2scores inthe observation group.Results①Platelet parameters:The control group had platelet count(PLT),mean plateletvolume(MPV)and platelet large cell ratio(P-LCR)respectively as(140.15±13.45)×109/L,(11.21±1.32)fl and(36.75±3.31)%,and the observation group had PLT,MPV and P-LCR respectively as(118.98±10.24)×109/L,(14.20±1.56)fl and(42.20±5.17)%.The observation group had lower PLT than the control group,while theother platelet parameters in the observation group were higher than the control group,and their differences allhad statistical significance(P<0.05).In the observation group,patients with higher ABCD2score had lower PLTthan those with low ABCD2score,while the other platelet parameters in them were all higher than those with lowABCD2score,and their differences all had statistical significance(P<0.05).②Platelet activation indexes:Thecontrol group had CD61,αgranule membrane protein-140(GMP-140)and platelet membrane glycoproteinⅡb/Ⅲa(GPⅡb/Ⅲa)respectively as(38.38±3.52)%,(8.07±0.72)%and(16.26±2.53)%,and the observationgroup had CD61,GMP-140and GPⅡb/Ⅲa respectively as(46.35±4.23)%,(13.34±1.35)%and(24.95±3.37)%.The observation group had all higher platelet activation indexes than the control group,and the differences all hadstatistical significance(P<0.05).In the observation group,patients with higher ABCD2score had all higher plateletactivation indexes than those with low ABCD2score,and the differences all had statistical significance(P<0.05).Conclusion Platelet-related parameters contains high detection value in patients with transient ischemic attack,and it shows certain guiding sig
