应用改良Twin-block功能矫治器治疗早期下颌后缩的临床研究

摘　　要：目的:分析改良Twin-block功能矫治器治疗儿童下颌后缩的侧貌改善临床效果及其应用价值。方法:选取安氏Ⅱ类1分类下颌后缩儿童23例为研究对象,对所有患儿行功能矫治器进行治疗,采用自身对照,比较分析治疗前后患儿相关的颜面美学指标变化。结果:经过矫治后,患儿的侧貌方面出现明显变化,经过治疗后患儿的SNA角79.05°、SNB角75.13°、ANB角4.11°、Z角63.47°、H角20.24°、上唇长:下唇长0.52、下唇-E线距1.76mm、颏颈角125.80°等指标明显优于治疗前Z角57.25°、H角24.36°、上唇长/下唇长0.61、下唇-E线距2.85mm、颏颈角121.35°,与术前比较差异有统计学意义(P<0.05)。结论:改良Twin-block功能矫治器治疗儿童下颌后缩能够有效地改善患者的颜面美学,并促使患者的颜面能够趋于协调、自然。Objective To analysis the clinical effect and application value of facial profile improvement of the modified Twinblockappliance in the treatment of children with mandibular retrusion.Methods To compare and analyze the changes offacial aesthetics indexs of children before and after treatment,our department selected23cases with Angle classⅡdivision1mandibular retrusion in children as the research subject.We carried out functional appliance treatment for all children and usethe self controlled manner.Results After treatment,patients with facial features especially noticeable changes in the profile.After the treatment of patients with SNA angle of79.05degrees,SNB angle of75.13degrees,ANB angle of4.11degrees,Zangle of63.47degrees,the H angle is20.24degrees,upper lip length/lower lip length0.52,lower lip-E line from1.76mm,side angle of125.80degrees which were significantly better than patients before treatment with Z angle of57.25degrees,the Hangle24.36degrees,upper lip length/lower lip length0.61,lower lip-E line from2.85mm,the side angle is121.35degrees.There is a significant difference compared with the preoperative(P<0.05),and the facial profile with great changes,were morelikely to accept,and gradually reach the normal value.Conclusion The modified Twin-block appliance in treatment of childrenwith mandibular retrusion can effectively improve the patient's facial aesthetics,and promote the patient's face to be moreharmonious,natural.
