食管曲张静脉出血硬化剂注射治疗的疗效及并发症研究

摘　　要：目的探究食管曲张静脉出血硬化剂注射治疗的治疗效果与并发症情况。方法 50例肝硬化食管静脉曲张破裂出血(EVB)患者,根据治疗方法的不同分为序贯硬化组和套扎组,各25例。在常规治疗的基础上,序贯硬化组患者加用聚桂醇-凝血酶-聚桂醇序贯硬化治疗,套扎组患者加用密集结扎法进行套扎治疗,比较两组治疗效果与并发症发生情况。结果两组患者静脉曲张完全消失率、有效率、再出血发生率比较,差异无统计学意义(P>0.05)。治疗后经过半年的随访发现,序贯硬化组患者复发率为4.00%(1/25)明显小于套扎组24.00%(6/25),差异有统计学意义(P<0.05)。套扎组患者的并发症发生率为16.00%,序贯硬化组患者并发症发生率为24.00%,两组比较差异无统计学意义(P>0.05)。两组患者均未出现死亡现象。结论序贯硬化治疗对食管曲张静脉出血的治疗方法有效,复发率较低,安全性较强。Objective To explore the treatment effect and complications of sclerosing agent injectiontherapy in esophageal variceal bleeding.Methods A total of50hepatocirrhosis patients with esophagealvariceal bleeding(EVB)were divided by different treatment methods into sequential sclerosis group and ligationgroup,with25cases in each group.On the basis of conventional therapy,the sequential sclerosis group receivedlauromacrogol-thrombin-lauromacrogol sequential sclerotherapy.The ligation group received intensive ligationmethod for ligation.Comparison were made on treatment effect and occurrence of complications in two groups.Results Both groups had no statistically significant difference in varicose veins completely disappearance rate,effective rate and incidence of rebleeding(P>0.05).After6months of follow-up,the sequential sclerosis grouphad obviously lower recurrence rate as4.00%(1/25)than24.00%(6/25)in ligation group,and the differencehad statistical significance(P<0.05).The ligation group had incidence of complications as16.00%,while thesequential sclerosis group had incidence of complications as24.00%,and the difference had no statisticalsignificance(P>0.05).There were no deaths in both groups.Conclusion Sequential sclerotherapy is effective inthe treatment of esophageal variceal bleeding with low recurrence rate and high safety.
