三维对比增强MRA窦汇区形态及颈内静脉引流形式分析

摘　　要：目的探讨窦汇区影像解剖形态和颈内静脉(internal jugular vein,IJV)引流形式。材料与方法头颈部动脉三维对比增强磁共振血管成像(3 dimensional contrast enhanced magnetic resonance angiography,3D CE-MRA)后加扫静脉期的1204例患者,男749例,女455例,年龄14~84岁,平均(65.11±11.99)岁。采用静脉期最大强度投影(maximum intensity projection,MIP)重建图像观察横窦(transverse sinus,TS)和IJV引流优势型及窦汇区形态类型。结果 1204例受检者中,TS引流右侧优势型占51.91%,左侧优势型占14.95%,等势型占33.14%。窦汇区解剖类型分为4型:简单型占14.20%,双分支型占31.48%,偏侧型占51.66%,枕窦型占2.66%。其中偏侧型根据上矢状窦(superior sagittal sinus,SSS)和直窦(straight sinus,SS)的不同偏侧方向又分为3个亚型。1162例受检者IJV引流右侧优势型占53.70%,左侧优势型占17.38%,等势型占28.92%。结论窦汇区影像解剖形态较为复杂,窦汇各分型的IJV引流形式和TS引流形式基本一致。Objective:To explore the imaging anatomy of torcular herophili and drainage dominant of internal jugular vein(IJV).Materials and Methods:A total of1204patients[749men,455women,age range,14-84years,mean age,(65.11士11.99)years]were included.Venous phase scanning was performed immediately after head and neck arterial phase scanning on3dimensional contrast enhanced magnetic resonance angiography(3D CE MRA).Transverse sinus(TS)and IJV drainage dominant types were observed with maximum intensity projection(MIP)images.Morphological types of the torcular herophili regions were classified.Results:Of the1204subjects,TS was found to be right,left,and codominant in51.91%,14.95%,and33.14%of the cases,respectively.According to anatomic features,torcular herophili regions were classified into4types:simple confluence type(14.20%),double bifurcation type(31.48%),dural venous sinuses deviation type(51.66%)and occipital sinus(OS)type(2.66%).The third type was subdivided into3subtypes according to the different deviations of the superior sagittal sinus(SSS)and the straight sinus(SS).Of the1162subjects,IJV was found to be right,left,and codominant in53.70%,17.38%,and28.92%of the cases,respectively.Conclusions:The morphological type of torcular herophili is rather complicated.In the different types of TS,the drainage form of IJV is consistent with that of TS drainage.
