垫式可摘局部义齿与固定义齿在重度磨耗伴缺损患者咬合重建中的应用比较

摘　　要：目的比较垫式可摘局部义齿与固定义齿对重度磨耗伴缺损的修复效果。方法将80例牙列重度磨耗伴缺损患者按随机数字表法随机分为2组,每组40例,A组采用垫式可摘局部义齿进行咬合重建,B组采用固定义齿进行咬合重建,修复前、后评估临床症状、咀嚼功能及患者满意度。结果修复后2组咀嚼功能评分比较差异无统计学意义(P>0.05);修复后B组颞下颌关节疼痛、食物嵌塞发生率高于A组(P<0.05);修复后A组对于舒适度、食物嵌塞满意度评分高于B组,而B组对稳固感满意度评分高于A组(P<0.05);A组继发龋发生率高于B组,而B组牙龈红肿发生率高于A组,但2组总并发症发生率比较差异无统计学意义(P>0.05)。结论 垫式可摘局部义齿与固定义齿咬合重建治疗均能有效改善重度磨耗伴缺损患者咬合功能,但前者修复舒适性更佳,且对颞下颌关节疼痛的改善效果较好,而后者修复更具稳固性,临床应根据患者需求合理选择。Objective To compare the occlusal splint removable denture with fixed denture repair effect of severe attrition and defect.Methods80patients with severe dental attrition and defect were randomly divided into2groups randomly,40cases in each group,group A was treated with occlusal splint removable denture.For occlusal reconstruction,B group with fixed denture occlusal reconstruction,repair,clinical symptoms evaluation,masticatory function and patient satisfaction.Results After the restoration of2groups of masticatory function score had no significant difference(P>0.05);B group after repair of temporomandibular joint pain,food impaction was higher than that in A group(P<0.05);After the restoration of A group for comfort,food impaction satisfaction score is higher than that of B group,B group of solid sense of satisfaction score is higher than that of A group(P<0.05);group A secondary caries incidence was higher than that of B group,B group and gingival swelling was higher than that in A group,but the2groups no difference in total complication rate statistical significance(P>0.05).Conclusion Occlusal splint removable denture with fixed denture occlusal reconstruction treatment can effectively improve the severe wear and defect with occlusal function,but the former is better to repair comfort,and temporomandibular joint pain improvement effect is good,and the latter to repair more stable according to the needs of patients with clinical.Logical choice
