评价血清CA125、CA199、CA153联合检测对肺癌诊断及分期的临床价值

摘　　要：目的评价血清CA125(糖链抗原125)、CA199(糖链抗原199)、CA153(糖链抗原153)联合检测对肺癌诊断及分期的临床价值。方法在2015年6月~2016年6月期间选取60例健康体检者、60例良性肺病患者、60例疑似肺癌患者为此次研究对象,对本次受检者进行CA199、CA125、CA153、CEA指标检测,再分析肿瘤标志物在判断患者病情、预后的重要性。结果肺癌患者CA199(135.48±25.36)U/m L、CA125(95.45±15.48)U/m L、CA153(21.45±3.69)U/mL、CEA(45.58±5.42)μg/mL均高于良性肺病组、健康体检组受检者,差异有统计学意义(P<0.05)。同时从诊断价值性角度分析,仅联合诊断特异性90.00%、敏感性98.00%、误诊率10.00%、漏诊率2.00%与病理检查结果无统计学意义(P>0.05);从TNM分期角度分析,肺癌患者Ⅳ期和Ⅲb期的CA199、CA125、CA153水平值明显高于Ⅲa期、Ⅱ期和Ⅰ期,差异有统计学意义(P<0.05)。结论血清CA125、CA199、CA153联合检测在肺癌诊断、临床分期中价值显著。Objective To evaluate clinical value of combined detection of serum CA125(carbohydrate antigen125),CA199(carbohydrate antigen199)and CA153(carbohydrate antigen153)in diagnosis and Staging of lung cancer.Methods60healthy cases,60patients with benign lung disease and60suspected lung cancer from June2015to June2016were selected as research objects.All subjects in this research were given detection of CA199,CA125,CA153and CEA.Significance of tumor marker in judging disease and prognosis of patients was analyzed.ResultsCA199,CA125,CA153and CEA of patient with lung cancer were respectively(135.48±25.36)U/mL,(95.45±15.48)U/mL,(21.45±3.69)U/mL and(45.58±5.42)ug/mL,all higher than those of the benign lung disease group and subjects inthe healthy cases group(P<0.05).By analyzing from the perceptive of diagnostic value,specificity90.00%,sensitivity98.00%,misdiagnosis rate10.00%and omission diagnostic rate2.00%of only combined detection had no differencewith pathological examination results(P>0.05).By analyzing from the perceptive of TNM staging,values ofCA199,CA125and CA153levels patients with lung cancer inⅣstage andⅢb stage,significantly higher than thoseinⅢa stage,Ⅱstage and I stage(P<0.05).Conclusion Combined detection of serum CA125,CA199and CA153in diagnosis and clinical staging of lung cancer has significant value.
