米索前列醇在剖宫产加子宫肌瘤剔除术后的应用

摘　　要：目的观察在行剖宫产加子宫肌瘤剔除术患者术后预防性使用米索前列醇的效果。方法选取2015年1月~2016年12月在本院住院的足月妊娠合并子宫肌瘤,并行剖宫产术加子宫肌瘤剔除术的患者60例为治疗组,另取同时期在本院住院的足月妊娠并单纯行剖宫产术的患者60例为对照组。治疗组在术毕予米索前列醇400μg置直肠,对照组无给药,观察记录两组产妇在术毕至产后2h期间总出血量及出现产后宫缩乏力出血的情况。结果治疗组产妇在术毕至产后2h内的出血量少于对照组,产后宫缩乏力出血的发生率低于对照组,差异有统计学意义(P<0.05)。结论术毕预防性给予米索前列醇直肠给药能有效地减少了患者产后2h内出血量及降低产后宫缩乏力出血发生率,适用于各级医院,尤其是基层医院,值得临床推广应用。Objective To observe the effect of prophylactic use of misoprostol after cesarean section plusmyomectomy.Methods60cases of full-term pregnancy complicated with uterine myoma,concurrent cesarean sectionplus myomectomy hospitalized in our hospital from January2015to December2016were selected as the treatmentgroup.Another60cases of term pregnancy and cesarean section at the same period in our hospital were selected ascontrol group.Patients in treatment group wre treated with misoprostol400ug rectum after operation.Patients in controlgroup were with no drugs.The total amount of bleeding and the incidence of postpartum uterine atony bleeding in thetwo groups of mothers in the2hours after the completion of surgery were observed and recorded.Results The amountof bleeding in the treatment group was less than that in the control group within2hours after operation,the incidence ofpostpartum uterine atony hemorrhage was lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion Prophylactic administration of misoprostol for rectal administration can effectively reducethe amount of bleeding within2hours after delivery and reduce the incidence of postpartum hemorrhage.It is suitablefor all levels of hospitals,especially the primary hospital.It is worthy of clinical application.
