彩超引导下EnCor微创旋切系统在临床触诊阴性乳腺病灶诊治中的应用

摘　　要：目的探讨彩超引导下EnCor微创旋切系统在临床触诊阴性乳腺病灶(NPBL)诊治中的应用价值。方法 200例NPBL病人共252个病灶,均在彩超引导下行EnCor微创旋切手术,标本送检。结果 252个NPBL中纤维腺瘤135个(53.6%),乳腺腺病77个(30.6%),乳腺囊肿25个(9.9%),乳管内乳头状瘤11个(4.4%),乳腺导管内癌3个(1.2%),浸润性导管癌1个(0.4%)。良性病变病人196例(98%),术后随访超声未见复发,4例恶性病变病人1例行保乳手术,其余3例行乳癌改良根治术,术后无肿瘤残留,低估率为0。术后发生局部血肿者8例,皮下血肿者12例,无气胸、伤口感染等其他并发症,皮肤切口愈合好。结论彩超引导下EnCor微创旋切手术能有效、微创、快速的确诊NPBL,即可降低早期乳腺癌的漏诊率,又有良好的美容效果,是临床诊治NPBL的首选方法之一。Objective To evaluate the application value of ultrasound guided EnCor vacuum-assisted biopsy system in nonpalpable breast lesions(N PBL).Methods200patients with NPBL(252lesions)were received ultrasound guided EnCor vacuum-assisted biopsy and pathological examination in the department of surgical oncology in suzhou municiple hospital.Results In the252lesions,135lesions(53.6%)were fibroadenoma,77lesions(30.6%)were mastopathy,25lesions(9.9%)were cyst,11lesions(4.4%)were intraductal papilloma,3lesions(1.2%)were intraductal carcinoma,1lesion(0.4%)was invasive ductal carcinoma.196cases(98%)were benign without positive finding follow-up.4cases were carcinoma made operation without residual tumors,the underestimated rate was0.Only8cases had the complication of hematoma and12cases had skin ecchymosis,the incision healed well,and received satisfied cosmetic effect.Conclusion As an effective,minimally invasive,and rapid diagnostic technique for NPBL,ultrasound guided Encor vacuum assisted biopsy can decrease the underestimated risk of breast carcinoma.Moreover,it can also receive satisfied cosmetic effect.It is one of the preferred treatment methods for NPBL.
