摘　　要：目的探究超声内镜引导下腹腔神经丛阻滞术在胰腺癌中的临床应用效果。方法从2013年9月~2016年9月期间本院收治的所有胰腺癌患者中随机选取其中的80例作为本次的研究对象,经伦理委员会通过将这80例患者分为对照组和研究组两组,对照组采用CT腹腔神经阻断术进行治疗;研究组采用超声内镜引导下腹腔神经丛阻滞术进行治疗,评估治疗后两组患者的胰腺癌疼痛控制效果,对比两组患者不同时间点的疼痛视觉模拟评分(VAS)和疼痛缓解度,对比两组患者的不良反应发生情况。结果进行治疗后,两组患者的VAS评分均有所下降,且研究组患者的VAS评分显著低于对照组,差异有统计学有意义(P<0.05);从疼痛缓解程度来看,研究组患者术后疼痛的缓解率显著高于对照组,差异有统计学有意义(P<0.05);从不良反应的发生情况来看,研究组患者的不良反应率显著低于对照组,差异有统计学有意义(P<0.05)。结论把超声内镜引导下腹腔神经丛阻滞术应用到胰腺癌的治疗和疼痛缓解当中,能够取得显著的治疗效果,可以有效缓解胰腺癌患者的疼痛程度,降低患者术后的不良反应发生率,具有临床推广的意义。Objective To explore the clinical application effect of endoscopic ultrasound guided celiac plexusblock in pancreatic cancer.Methods80pancreatic cancer patients were randomly selected from all the pancreaticcancer patients cured in our hospital from September2013to September2016.Through the ethics committee,the80patients were divided into two groups:the control group and the study group.Patients in control group were treatedwith CT celiac nerve block,and patients in study group were treated with celiac plexus block guided by endoscopicultrasound.Pancreatic cancer pain control effect of the two groups was assessed.Pain visual analogue scales(VAS),pain relief,and adverse events at different times of the two groups were compared.Results After treatment,theVAS score of the two groups decreased,and the VAS score of the patients in the study group was significantly lowerthan that of the control group(P<0.05),it was statistically significant.From the degree of pain relief,the reliefrate of postoperative pain in the study group was significantly higher than that in the control group(P<0.05),it wasstatistically significant.From the incidence of adverse reactions,the adverse reactions rate of patients in the studygroup was significantly lower than that in the control group,it was statistically significant(P<0.05).ConclusionEndoscopic celiac plexus block guided by endoscopic ultrasonography used in the treatment of pancreatic cancerand pain relief can achieve significant therapeutic effect,can effectively alleviate the degree of pain in patients withpancreatic cancer,and reduce the incidence of postoperative ad verse reactions.It is worthy of clinical promotion.
