异基因造血干细胞移植治疗成人T细胞急性淋巴细胞白血病的临床效果分析

摘　　要：目的分析异基因造血干细胞移植治疗成人T细胞急性淋巴细胞白血病的临床效果。方法选择2006年1月~2016年12月76例成人T细胞急性淋巴细胞白血病患者根据随机数字表分组,各38例。标准组采用标准异基因造血干细胞移植治疗,强化组实施强化异基因造血干细胞移植治疗。比较两组T细胞急性淋巴细胞白血病存活率;生存时间、血小板(PLT)重建时间、中性粒细胞绝对数(ANC)重建时间;患者复发率、死亡率。结果强化组T细胞急性淋巴细胞白血病存活率高于标准组(P<0.05);强化组生存时间显著长于标准组(P<0.05);强化组患者PLT重建时间、ANC重建时间和标准组相近(P>0.05);强化组复发率、死亡率低于标准组(P<0.05)。结论异基因造血干细胞移植治疗成人T细胞急性淋巴细胞白血病的临床效果确切,可有效改善患者病情,延长生存时间,降低复发率和死亡率,值得借鉴。Objectvie To analyze the clinical effect of allogeneic hematopoietic stem cell transplantation fortreatment of adult T cell acute lymphoblastic leukemia.Methods76adults patients with T cell acute lymphoblasticleukemia cured in our hospital from January2006to December2016were selected and randomly divided into twogroups according to random digital table method with38cases in each.Patients in standard group were with standardallogeneic hematopoietic stem cell transplantation,and patients in reinforcement group were treated with allogeneichematopoietic stem cell transplantation.T cell acute lymphoblastic leukemia survival rate,survival time,platelet(blood,platelet,PLT)reconstruction time,neutrophil absolute number(ANC:absolute,neutrophil,count),reconstruction time,patient recurrence and mortality of the two groups were compared.Results The survival rate of acute lymphoblasticleukemia in T group was higher than that in standard group,P<0.05.The survival time of the intensive group wassignificantly longer than that of the standard group,P<0.05.The time of PLT reconstruction and the time of ANCreconstruction in the intensive group were similar to that of the standard group,P>0.05.The recurrence rate andmortality rate of the intensive group were lower than that of the standard group,P<0.05.Conclusion Allogeneichematopoietic stem cell transplantation is effective in the treatment of adult T cell acute lymphoblastic leukemia.Itcan effectively improve the patient's condition,prolong the survival time,reduce the recurrence rate and mortalityrate,and is worth learning.
