姑息切除联合化疗治疗晚期胃癌的临床研究

摘　　要：目的探讨姑息切除联合化疗治疗有并发症的晚期胃癌的临床疗效。方法回顾性收集2010年1月至2014年12月收治的42例并发出血或幽门梗阻等并发症的晚期胃癌患者,接受姑息性切除手术和改良DCF方案化疗,至少完成2个周期治疗后评估临床疗效及不良反应,并通过随访观察患者长期生存的时间。结果 9例患者行姑息性全胃切除术,33例行姑息性远端胃切除术。术后行改良DCF方案化疗3~11个周期,不良反应主要为骨髓抑制、脱发及恶心、呕吐等胃肠道反应。中位生存时间为14.1个月,1年生存率为61.1%,3年生存率为20.5%。结论对于并发出血、幽门梗阻的晚期胃癌,姑息性切除术后联合改良DCF方案化疗具有较好临床疗效,毒副反应可以耐受。Objective To investigate the clinic value of palliative gastrectomy combined with chemotherapy for advanced gastric cancer with complications.Methods From January2010to December2014,a total of42patients who underwent palliative gastrectomy combined with modified DCF regime chemotherapy due to advanced gastric cancer with complications of pyloric obstruction or bleeding were chosen in the hospital.Each patient received at least2cycles of the treatment,and the efficacies and toxicities were evaluated after treatment.The long-term survivals were evaluated by following-up.Results9patients were performed palliative total gastrecomy and33patients were performed distal sub-gastrectomy,respectively.The patients received3to11cycles of modified DCF regime after surgery.The main adverse reactions were myelosuppression,alopecia and gastrointestinal reactions such as nausea and vomiting.The median survival time was14.1months.The1-year and3-year survival rates were61.1%and20.5%,respectively.Conclusion For advanced gastric cancer with complications such as pyloric obstruction and bleeding,the palliative gastrectomy combined with modified DCF regime chemotherapy are efficacious,the toxicities and adverse reactions are tolerable.
