PP方案与TP方案治疗晚期肺腺癌的近期疗效对照研究

摘　　要：目的对照研究培美曲塞联合顺铂(PP方案)与紫杉醇联合顺铂(TP方案)治疗晚期肺腺癌的近期疗效。方法选取诊治的60例完全肺腺癌患者,随机分为PP方案组与TP方案组,分别采用培美曲塞联合顺铂、紫杉醇联合顺铂方案化疗,2个化疗周期后观察近期治疗疗效,观察化疗不良反应。结果 PP方案组治疗有效率、疾病控制率分别为53.33%、73.33%、高于TP方案组的26.67%、46.67%,比较差异有统计学意义(P<0.05);PP方案组骨髓抑制、胃肠道反应、肝肾功能损害、神经毒性发生率分别为20.00%、33.33%、6.67%、3.33%,TP方案组分别为46.67%、60.00%、10.00%、10.00%,PP方案组骨髓抑制及胃肠道反应发生率低于TP方案组(P<0.05),PP方案组Ⅲ~Ⅳ级骨髓抑制及胃肠道反应发生率为6.67%、10.00%,低于TP方案组26.67%、33.33%(P<0.05)。结论治疗完全肺腺癌PP方案近期疗效优于TP方案,骨髓抑制及胃肠道反应相对较轻。Objective To study the short-term efficacy of combination chemotherapy with paclitaxel and cisplatin(PP regimen)combined withpaclitaxel and cisplatin(TP regimen)in the treatment of advanced lung adenocarcinoma.Methods60cases of complete lung adenocarcinoma patientsand methods of diagnosis and treatment,were randomly divided into PP group and TP group,respectively with pemetrexed combined with cisplatinand paclitaxel combined with cisplatin chemotherapy,therapeutic efficacy after2cycles of chemotherapy,observe the adverse reaction of chemotherapy.Results PP group effective rate,disease control rate were53.33%and73.33%,higher than the TP group26.67%,46.67%,the differencewas statistically significant(P<0.05);PP group of bone marrow suppression,gastrointestinal reaction,liver and kidney dysfunction,the incidence ofneurotoxicity were20%,33.33%,6.67%,3.33%,TP group were46.67%,60%,10%,10%,PP were bone marrow suppression and gastrointestinal reactionrate is lower than the TP group(P<0.05),PP group of grade III-IV myelosuppression and gastrointestinal reaction rate was6.67%,10%,lowerthan26.67%in the TP group,33.33%(P<0.05).Conclusion The treatment of complete lung adenocarcinoma PP regimen is better than the TP regimen,bone marrow suppression and gastrointestinal reaction is relatively light.
