海洛因依赖者脱毒治疗并发十二指肠溃疡穿孔1例

摘　　要：我们在对海洛因依赖者脱毒治疗过程中发现,有1例患者并发十二指肠溃疡穿孔,并对其进行分析。患者入所后予纳洛酮脱毒治疗,治疗3个月后基本戒除毒瘾。但治疗2周后,患者临床表现为黑便、呕血、腹部不适、疼痛和头晕等。经过胃镜检查表明为十二指肠溃疡穿孔。14C-尿素呼气试验检查结果表明患者十二指肠溃疡穿孔与幽门螺杆菌无关。经泮托拉唑静脉滴注治疗,结果显示治疗后,患者呕血现象在1~2 d内停止,柏油样便4 d内转为黄色,生命体征平稳;患者脉搏从治疗前的90次/min左右减少至80次/min左右。患者治疗前的胃液p H值均1.8左右,治疗后患者的胃液p H值升高为5.6。治疗效果较优。In our detoxification treatment of heroin addicts,1patient had perforated duodenal ulcers and was analyzed.After thepatient was admitted,the case was treated with naloxone.After3months treatment,they basically got rid of drug addiction.But after2weeks of treatment,the clinical manifestations were hematemesis,melena,abdominal discomfort,pain and dizziness.Aftergastroscopy,it showed perforation of duodenal ulcer.14C-urea breath test showed that perforation of duodenal ulcer had nothingto do with Helicobacter pylori.After intravenous pantoprazole treatment,the results showed that after treatment,patients withhematemesis phenomenon in1～2days to stop,tarry stool turn yellow within4days,with stable vital signs.The patient's pulsedropped from about90/min before treatment to about80times/min.The PH of gastric juice before treatment was about1.8,and thegastric juice PH increased by5.6after treatment.The treatment effect is better.
