青少年配戴角膜塑形镜安全性的临床观察

摘　　要：目的:观察青少年近视患者配戴角膜塑形镜的安全性。方法:选择长期配戴角膜塑形镜的60名(120眼)青少年近视患者作为研究对象,观察治疗2年后眼表并发症、中央角膜厚度(CCT)和角膜内皮细胞各项指标的改变。结果:戴镜前后CCT参数比较无统计学意义(P=0.08);戴镜前后角膜内皮细胞各项指标参数比较无统计学意义(P>0.05);角膜点染在戴镜后1 d、1周、1个月、1年、2年的发生率分别为29.17%(35/120)、15.83%(19/120)、5%(6/120)、0.83%(1/120)、2.5%(3/120);角膜压痕3例,占2.5%(3/120),结膜反应性充血4例,占3.33%(4/120),无结膜滤泡、乳头,角膜水肿,角膜新生血管和上皮微囊,未发生严重角膜感染及护理液过敏反应。结论:青少年近视患者长期配戴角膜塑形镜无严重角膜、结膜并发症,对CCT和角膜内皮细胞影响甚微,安全性较高。Objective:To observe the security of orthokeratology for teengers myopia correction.Methods:60(120eyes)myopi-a teenagers with long-term wearing orthokeratology were selected for this study.Ocular surface complication,central corneal thickness(CCT)and corneal endothelial cell changes were observed during the two years of wearing.Results:CCT and the corneal endothelial cell did not show significantly changes after wearing orthokeratology(P=0.08,P>0.05),there was no statistical significance.1day,l week,1month,1year and2years after wearing orthokeratology,the rate of positive corneal fluorescein staining was29.17%(35/120),15.83%(19/120),5%(6/120),0.83%(1/120),2.5%(3/120)Respectively.3cases had corneal impression,accounting for2.5%(3/120).4cases were suffered from conjunctival hyperemia,accounting for3.33%(4/120).No conjunctival follicles,papillae,corne-al edema,corneal neovascularization and epithelial microcapsules were observed.And no severe corneal infection and allergic reactions of the nursing fluids were found.Conclusion:It is safe that the myopia teenagers wear orthokeratology for a long time.Severe corneal and conjunctival complications will not happen,and wearing orthokeratology just has little effect on CCT and corneal endothelial cells,and has high safety.
