急性胰腺炎治疗前后胰胆管汇合夹角的MRCP研究

摘　　要：目的:通过MRCP对比研究不同严重程度的急性胰腺炎患者治疗前、后胰胆管汇合夹角的变化。方法:收集2015年2月至2016年7月本院确诊的急性胰腺炎患者87例,同一患者在治疗前后均行MRCP检查,根据纳入标准分为A、B两组,A组为治疗前,B组为治疗后。按照中国急性胰腺炎诊治指南将所有患者分为轻症急性胰腺炎(mild acute pancreatitis,MAP)和重症急性胰腺炎(severe acute pancreatitis,SAP)。采用两配对t检验对A组和B组的胰胆管汇合夹角进行统计分析,采用两独立样本t检验分析MAP和SAP在治疗前、治疗后的胰胆管汇合夹角。结果:急性胰腺炎患者治疗前胰胆管汇合夹角平均值(63.36±16.17)°,治疗后胰胆管汇合夹角平均值(63.11±15.74)°,A、B两组胰胆管汇合夹角无统计学意义(P>0.05);治疗前MAP和SAP的胰胆管汇合夹角无统计学意义(P>0.05),分别为(61.29±16.69)°和(66.58±15.01)°;治疗后MAP和SAP的胰胆管汇合夹角无统计学意义(P>0.05),分别为(61.06±16.30)°和(66.31±14.47)°。结论:急性胰腺炎治疗前、后胰胆管汇合夹角没有明显变化,急性胰腺炎的严重程度和胰胆管汇合夹角大小没有相关性。Objective:To investigate the correlation between the angle of pancreaticobiliary junction and the acute pancreatitis with different severity using magnetic resonance cholangiopancreatography(MRCP).Methods:87cases of acute pancreatitis diagnosed in our hospital from February2015to July2016were collected,MRCP examinations were performed in patients between pre and post treatment.87patients were enrolled into our study according to inclusion and exclusion criteria,and divided into Group A(pre treat-ment)and B(post treatment)Respectively.According to the guidelines of diagnosis and treatment of acute pancreatitis in China,all pa-tients were divided into mild acute pancreatitis(MAP)and severe acute pancreatitis(SAP).The angles of the pancreaticobiliary junc-tion between group A and B were compared by two paired t-tests.The angles of MAP and SAP before and after treatment of acute pan-creatitis were compared by using two independent samples t-test.Results:The average angle of the pancreaticobiliary junction in Group A was63.36°±16.17°,and Group B was63.11°±15.74°.There was no significant difference between Group A and B(P>0.05).In pre treatment,the angles of MAP and SAP were no significant difference(P>0.05),and the average values of the angle of the pancre-aticobiliary junction were61.29°±16.69°and66.58°±15.01°,respectively.In post treatment,the angles of MAP and SAP were no significant difference(P>0.05),and the average values of the angle of the pancreaticobiliary junction were61.06o±16.300and66.310±14.47°Respectively.Conclusion:There is no change in angle of the pancreaticobiliary junction between pre and post treatment of acute pancreatitis.And there is no relationship between the severity of acute pancreatitis and angle of the pancreaticobiliary junction.
