D-D、FIB、PLT等凝血功能指标与肺癌患者外周血CTC及病理特征的相关性研究

摘　　要：目的探讨D-二聚体(D-D)、纤维蛋白原(FIB)、血小板(PLT)等凝血功能指标与肺癌患者外周血中循环肿瘤细胞(CTC)、病理特征的关系。方法选取我院经病理学确诊的肺癌患者118例作为研究组,选取同期在我院进行体检的健康志愿者60例作为对照组,分别检测两组D-D、FIB、PLT、活化部分凝血酶时间(APTT)、凝血酶原时间(PT)以及研究组的外周血CTC,并进行比较分析。结果研究组患者D-D、FIB、PLT、PT水平均显著高于对照组(P<0.05),APTT水平显著低于对照组(P<0.05);Ⅲ+Ⅳ期肺癌患者D-D、FIB、PLT、PT水平均显著高于Ⅰ+Ⅱ期肺癌患者(P<0.05),APTT水平显著低于Ⅰ+Ⅱ期肺癌患者(P<0.05);CTC阳性肺癌患者D-D、FIB、PLT、PT水平均显著高于CTC阴性肺癌患者(P<0.05),APTT水平显著低于CTC阴性肺癌患者(P<0.05);肺癌分期越高,患者CTC阳性表达率越高,CTC阳性率与肺癌分期呈显著正相关(r=0.622,P<0.05)。结论肺癌患者血栓形成风险增加,并且各项凝血功能指标与TNM分期、外周血CTC具有一定的相关性。Objective To investigate the relationship of D-D,FIB,PLT,other coagulation parameters and peripheral blood CTC,pathologicalfeatures in patients with lung cancer.Methods118lung cancer patients were selected by pathologically confirmation as the studygroup,60healthy volunteers were selected as control group,D-D,FIB,PLT,activated partial thromboplastin time(APTT),prothrombintime(PT),CTC in the peripheral bloodof patients in the two groups were analyzed.Results D-D,FIB,PLT and PT levels of patients in studygroup were significantly higher than those in the control group(P<0.05);the APTT level of patients in study group was significantly lowerthan that of control group(P<0.05);D-D,FIB,PLT and PT levels of lung cancer patients at stage III/IV were significantly higher than thoseat stage I/II patients with lung cancer(P<0.05);APTT levels of lung cancer patients at stage III/IV were significantly higher than those atstage I/II patients with lung cancer(P<0.05);D-D,FIB,PLT,PT levels of patients with CTC positive lung cancer were significantly higherthan that in patients with negative CTC lung cancer(P<0.05);the APTT level of patients with CTC positive lung cancer was significantlylower in CTC negative patients with lung cancer(P<0.05);the higher the staging of lung cancer was,the higher rate of the positive expressionof CTC in patients was.The positive rate of CTC and staging of lung cancer showed a significant positive correlation(r=0.622,P<0.05).Conclusion The risk of coagulation increase in patients with lung cancer,and the coagulation parameters were related to the TNM stage andperipheral blood CTC.
