不同强度高频重复经颅磁刺激对慢性精神分裂症执行功能的影响

摘　　要：目的探讨不同强度高频经颅磁刺激(r TMS)对慢性精神分裂症患者执行功能的影响。方法将60例慢性精神分裂症患者随机分为两组,在原抗精神病药基础上,分别联用110%或80%运动阈值的高频r TMS,每周5次,疗程6周。在基线期和治疗末采用威斯康星卡片分类测验(WCST)、Stroop色词测验(SCWT)及连线测验B(TMT-B)评估疗效,副反应量表(TESS)评估安全性。结果在治疗末,两组中WCST正确应答数(Rc)、完成分类数(Cc)、完成第1个分类所需应答数(Rf)、持续性错误数(Rpe)、Stroop-CW及TMT-B均低于基线期(t=-2.07、-2.42、2.30、2.15、6.92、4.60,t=-3.66、-3.00、2.64、12.29、10.45、4.39;均P<0.05或P<0.01)且高强度组Rpe低于低强度组(t=2.05,P<0.05)。两组间不良反应比较差异无统计学意义。结论高频r TMS能有效治疗慢性精神分裂症患者执行功能障碍,安全性好且高刺激强度疗效更佳。Objective To explore the efficacy and safety of antipsychotics combined with different intensity high-frequency repetitive transcranial magnetic stimulation(rTMS)in the treatment of executive function of chronic schizophrenia.Methods60chronic schizophrenia inpatients were randomly divided into two groups for6weeks.One group was antipsychotics combined with80%motor threshold(MT),the other was antipsychotics combined with110%MT.Wisconsin card sorting test(WCST),Stroop colorword test(SCWT),Trail making test B(TMT-B)and Treatment Emergent Symptoms Scale(TESS)were used to assess the efficacy and safety at baseline and3th,6th week after treatment.Results At the end of 6th week,total correct responses of WCST(RC),categories completed of WCST(Cc),trials to compete first category of WCST(RF),perseverative responses errors of WCST(Rpe),color word test of SCWT(Stroop-CW)and TMT-B in the two groups were lower compaired with those of the baseline assessment(t=-2.07,-2.42,2.30,2.15,6.92,4.60;t=-3.66,-3.00,2.64,12.29,10.45,4.39;all P<0.05or P<0.01),and Rpe in110%MT group were lower than those in80%MT group(t=2.05,P<0.05).There was no significant difference in side effects between two groups.Conclusion High-frequency rTMS is effective and safe in the treatment of executive function of chronic schizophrenia,and the high-stimulus intensity is better.
