卡培他滨维持化疗治疗晚期胃癌和术后复发转移胃癌的疗效评价

摘　　要：目的研究卡培他滨维持化疗治疗晚期胃癌和术后复发转移胃癌的临床疗效。方法选取2012年3月～2015年8月天津市第五中心医院收治的晚期胃癌与术后复发转移胃癌患者82例，按照随机数字表法均分成研究组与对照组，每组各41例。对照组予以常规对症治疗，研究组则在对照组的基础上予以口服卡培他滨维持化疗治疗。对2组患者均进行为期2年的随访观察。分别比较2组近期疗效，治疗后血清CEA、CA199以及CA125水平，不良反应发生情况以及1年、2年生存率。结果研究组有效率明显高于对照组（P＜0.05）。治疗后研究组血清CEA、CA199以及CA125水平分别明显低于对照组（均P＜0.05）。2组静脉炎、白细胞下降、肾功能损害以及神经毒性发生率比较无统计学意义。治疗后研究组1年生存率、2年生存率分别明显高于对照组（均P＜0.05）。结论卡培他滨维持化疗治疗晚期胃癌和术后复发转移胃癌的临床疗效显著，可有效改善患者血清CEA、CA199以及CA125水平，提高患者生存率。Objective To study the effect of capecitabine maintenance chemotherapy in the treatment of advanced gastric cancer recurrence and metastasis of gastric carcinoma.Methods82patients with advanced gastric cancer or a postoperative recurrence and metastasis of gastric carcinoma From March2012to August2015were randomly divided into study group and control group,41cases in each group.The control group was given routine symptomatic treatment,the study group was given on the basis of the control group with capecitabine maintenance chemotherapy treatment.Two groups of patients were followed up for a period of2years.After treatment,the efficacy,serum CEA,CA199and CA125levels,the incidence of adverse reaction and1years,2years survival rate were compared between two groups.Results The study group of total efficacy was significantly higher than the control group(P<0.05).After treatment,the study group of serum CEA,CA199and CA125levels were significantly lower than the control group(P<0.05).Two groups of phlebitis,leukopenia,renal impairment and neurotoxicity were compared,had no statistical significance.The study group of1years survival rate after treatment,2year survival rates were significantly higher than the control group(P<0.05).Conclusion Capecitabine maintenance chemotherapy in the treatment of advanced gastric cancer recurrence and metastasis of gastric cancer of clinical curative effect was significantly,can effectively improve the patients serum CEA,CA199and CA125level,increase the survival rate of the patients.
