膀胱肉瘤样癌的病理诊断和临床分析

摘　　要：目的探讨膀胱肉瘤样癌的病理检查情况及相关临床特征，为临床提高此类疾病诊治效果提供可靠依据。方法回顾性分析松阳县人民医院2015年2月～2017年4月收治的13例膀胱肉瘤样癌患者临床资料，内容包括病理检查结果（临床分型、浸润程度等）、临床表现、病灶情况（部位、最大直径等），将所得数据输入SPSS软件后给予相应分析并得出结论。结果13例膀胱肉瘤样癌患者临床表现以间断性无痛性肉眼血尿为主（100.00%），发病部位多位于膀胱侧壁（53.85%），高达53.85%患者病灶最大直径在4cm～6cm范围内，病理检查结果可见以移行细胞癌所占比例最高（69.23%），46.15%患者浸润程度不详。结论膀胱肉瘤样癌临床较为少见但预后差，临床医生应对此病给予一定重视，接诊疑似病例后根据其各项检查综合判断病情，从而有利于保障患者疗效、预后。Objective To investigate the pathological examination and related clinical features of sarcomatoid carcinoma of bladder,and to provide reliable evidence for improving the diagnosis and treatment of this disease.Methods A retrospective analysis of the clinical data of13cases of bladder sarcomatoid carcinoma patients treated in Songyang People’s Hospital from February2015to April2017,including the results of pathological examination(clinical type,degree of infiltration etc.),clinical manifestation,lesion(maximum diameter position,etc.),the data input SPSS software to give the corresponding analysis and draw the conclusion.Results13cases of sarcomatoid carcinoma of the urinary bladder in patients with clinical manifestations with intermittent painless gross hematuria(100.00%),the incidence site located in the lateral wall of bladder(53.85%),up to53.85%of patients with the maximum diameters were in the range of4-6cm,the pathological examination results observed in transitional cell carcinoma of the highest proportion(69.23%),46.15%with the infiltration degree of unknown.Conclusion Sarcomatoid carcinoma of the bladder is rare,but the prognosis is poor.Clinicians should pay attention to this disease.After receiving the suspected cases,the disease should be judged according to its comprehensive examination,so as to ensure the curative effect and prognosis of the patients.
