摘 要:目的:比较扫频CASIA SS-1000 OCT与Sirius眼前节分析仪两种仪器测量正常人角膜顶点厚度(CCT)和最薄点角膜厚度(TCT)结果的差异性、相关性及一致性,为临床的应用提供理论依据。方法:前瞻性研究。收集近视患者34例,均由同一熟练操作者对受检者右眼进行测量,先采用Sirius眼前节分析仪,再使用SS-1000 OCT重复测量角膜厚度。配对t检验及Bland-Altman用于评估SS-1000 OCT与Sirius眼前节分析仪的角膜顶点及角膜最薄点的一致性。结果:SS-1000 OCT及Sirius角膜地形图仪测量CCT的角膜顶点均值分别为517.62±25.29、518.47±27.23μm。SS-1000 OCT及Sirius眼前节分析仪CCT的角膜最薄点均值分别为513.53±25.06、515.32±26.69μm。两者角膜顶点角膜厚度比较,差异无统计学意义(P>0.05),但两者角膜最薄点的角膜厚度比较,差异具有统计学意义(P<0.05)。两种设备测量的角膜厚度有较好的相关性(r=0.969、0.965,均P<0.05)。做Bland-Altman分析,角膜顶点厚度的95%一致性界限为(-14.22μm,12.52μm),角膜最薄点厚度的95%一致性界限为(-15.61μm,12.03μm),4%(3/68)的点在95%一致性的界限外,但角膜最薄点的一致性差异更大。结论:SS-1000 OCT测量CCT与Sirius眼前节分析仪具有高度的一致性,在临床工作中,可以考虑相互替代。AIM:To compare CASIA SS-1000and Sirius OCT sweep anterior segment analyzer instrument for measuring normal corneal vertex thickness(CCT)and the thinnest corneal thickness(TCT)results the difference,correlation and consistency,and provide a theoretical basis for clinical application.METHODS:This was a prospective study.A total of34normal subjects were collected.The subjects were measured by the same skilled operator.The SS-1000OCT was first used,and then the corneal thickness was measured repeatedly by Sirius anterior segment analyzer.Paired f test and Bland-Altman were used to evaluate the consistency of corneal apex and corneal thinnest point between SS-1000OCT and Sirius anterior segment analyzer.RESULTS:The mean corneal apex measured by SS-1000OCT and Sirius corneal topography were517.62±25.29m m and518.47±27.23m m CCT,respectively.The thinnest points of SS-1000OCT and Sirius anterior segment analyzer GCH'were513.53±25.06m m and515.32±26.69m m,respectively.Paired t test showed that the difference on corneal thickness of vertex was not statistically significant(P>0.05),but the thinnest corneal thickness was statistically significant(P<0.05).Pearson analysis of the two devices,the correlation is0.969,0.965.The results of95%consistency limiting analysis on the corneal vertex thickness by Bland-Altman was(-14.22mm,12.52pm),that of the thinnest corneal thickness was(-15.61nm,12.03Mm),4%(3/68)was out of the95%consistency limiting,but the thinnest corneal thickness was of a little larger differences.CONCLUSION:SS-1000OCT measurement of CCT and Sirius anterior segment analyzer is highly consistent,in clinical work can be considered alternative,but the thinnest point of the cornea can not be replaced each other.
