新疆石河子地区三阴性与非三阴性乳腺癌临床病理特征及预后分析

摘　　要：目的:分析新疆石河子地区三阴性乳腺癌(triple-negtive breast cancer,TNBC)与非TNBC临床病理特征及预后。方法:回顾性分析新疆石河子地区女性乳腺癌患者360例,手术病理资料完整,比较TNBC和非TNBC的临床病理特征、复发、转移及生存分析。结果:TNBC较非TNBC具有以下特征:肿瘤直径较大(肿瘤直径≥2cm分别是78.3%、37.6%,P<0.05),淋巴结转移率高(73.3%、53.0%,P<0.05);淋巴结转移数目多(淋巴结转移数≥4个分别是36.7%、13.9%,P<0.05);临床分期晚(Ⅲ-Ⅳ期比例分别是46.7%、24.3%,P<0.05);容易发生肺转移(21.7%、5.7%,P<0.05);TNBC的5年生存率是75.2%,非TNBC的是86.7%(P<0.05);TNBC的5年无疾病生存率是52.5%,非TNBC的是75.5%(P<0.05)。结论:新疆石河子地区TNBC患者具有肿瘤大、淋巴结转移多、临床分期晚的特征,且易发生肺转移、预后差。Objective:To analyze the clinical characteristics and survival time of patients with triple-negtive breast cancer(TNBC)and non-triple-negative breast cancer(non-TNBC)in Shihezi region of Xinjiang.Methods:360cases of female breast cancer patients were collected into this retrospective study,these patients all came from Shihezi region.Clinical characteristics,recurrence and metastatic time,and overall survival time were compared between TNBC and non-TNBC groups.Results:Compared with non-TNBC group,TNBC had following characteristics:larger tumor size(the ratio of the tumor size≥2cm were78.3%and37.6%respectively,<0.05),higher lymph node metastasis ratio(73.3%and53.0%,<0.05),more metastatic lymph nodes(the number of lymph nodes≥4were36.7%and13.9%,respectively,<0.05),later clinical stage(Ⅲ-Ⅳstage rates were46.7%and24.3%,<0.05),propensity for lung metastases(21.7%and5.7%,<0.05).The five-year overall survival rates of TNBC were75.2%,and non-TNBC were86.7%(<0.05).The five-year disease free survival rates of TNBC were52.5%,and non-TNBC were75.5%(<0.05).Conclusion:TNBC patients had larger tumor size,more metastatic lymph nodes,later clinical stage,propensity for lung metastasis,poor survival.
