摘　　要：目的:探讨腹腔镜手术治疗对结直肠癌患者血清炎性因子、血管内皮生长因子(VEGF)、基质金属蛋白酶-9(MMP-9)、氧化应激及免疫功能的影响。方法:103例结直肠癌患者依据随机数据表法分为对照组(n=52)和观察组(n=51),对照组患者给予传统开腹手术治疗,观察组给予腹腔镜手术治疗,比较术前及术后第1d两组血清炎性因子、VEGF、MMP-9、氧化应激及免疫功能水平。结果:术前,两组TNF-α、IL-6、CRP、VEGF、MMP-9、MDA、SOD、CD4^+、CD8^+、CD4^+/CD8^+水平比较,差异无统计学意义(P>0.05)。术后第1d,两组TNF-α、IL-6、CRP、MDA及CD8^+水平均显著高于组内术前水平,且观察组显著低于对照组,差异有统计学意义(P<0.05);两组VEGF、MMP-9、SOD、CD4^+、CD4^+/CD8^+水平均显著低于组内术前,且观察组中VEGF、MMP-9水平均显著低于对照组,SOD、CD4^+、CD4^+/CD8^+水平均显著高于对照组,差异有统计学意义(P<0.05)。结论:与开腹手术相比,腹腔镜手术治疗结直肠癌,可有效减少炎性因子的释放,降低VEGF、MMP-9水平,对患者的氧化应激及免疫功能的影响更小,具有一定的临床价值。Objective:To investigate the effects of laparoscopic surgery on serum inflammatory factors,vascular endothelial growth factor(VEGF),matrix metalloproteinase9(MMP9),oxidative stress and immune function in patients with colorectal cancer.Methods:According to the random data table103cases of colorectal cancer patients were divided into the control group(n=52)and observation group(n=51),patients in the control group were treated with traditional open surgery,and observation group patients were given laparoscopic surgery,preoperative and postoperative1d,the levels of serum inflammatory factor,VEGF,MMP9,oxidative stress and immune function of the two groups were compared.Results:There were no significant differences in TNFα,IL6,CRP,VEGF,MMP9,MDA,SOD,CD4+,CD8+,CD4+/CD8+levels between the two groups before operation(P>0.05).Postoperative1d,the levels of TNFα,IL6,CRP,MDA and CD8+in the two groups were significantly higher than those in the same group preoperative,and the observation group was significantly lower than the control group,the difference was statistically significant(P<0.05);Two groups of VEGF,MMP9,SOD,CD4+and CD4+/CD8+were significantly lower than those in the group before the operation,and the observation group VEGF,MMP9levels were significantly lower than those in the control group,SOD,CD4+,CD4+/CD8+levels of the observation group were significantly higher than the control group,the difference was statistically significant(P<0.05).Conclusion:Compared with open surgery,laparoscopic surgery for colorectal cancer,can effectively reduce the release of light inflammatory factors,decrease VEGF,MMP9levels,less influence on oxidative stress and immune function of patients,has a certain clinical value.
