男性鼻咽癌患者调强放射治疗时性腺的散射线剂量研究

摘　　要：目的探讨男性鼻咽癌患者调强放射治疗时性腺的散射线剂量。方法选取2015年4月10日至2015年9月19日该院肿瘤中心收治的经影像学诊断和病理检查确诊为鼻咽癌放射治疗男性患者28例。测量所有患者性腺离射野的距离,利用巡测剂量仪测量模拟影像引导验证和治疗条件下性腺位置的散射线剂量。结果性腺到射野距离51~64 cm,平均(55.29±0.53)cm;锥形束CT(CBCT)影像引导验证过程中接受的平均剂量为(0.53±0.01)mSv,其有效剂量为(0.11±0.01)mSv;在治疗过程中接受的平均剂量为(74.57±1.01)mSv,其有效剂量为(14.91±0.20)mSv;整个疗程中接受的平均剂量为(75.09±1.00)mSv,其有效剂量为(15.02±0.20)mSv。结论男性鼻咽癌患者在接受调强放射治疗时,性腺接收到较低水平的散射线剂量,应在行调强放射治疗时对这一放射敏感器官予以充分评估。Objective To investigate the scattered radiation dosage of sexual gland in the nasopharyngeal carcinoma in鄄tensity鄄modulated radiotherapy.Methods Twenty鄄eight male patients with nasopharyngeal carcinoma diagnosed by imagiology and confirmed by pathology in the tumor center of the Affiliated Tongji Hospital of Medical College of Huazhong University of Sci鄄ence and Technology from April10袁2015to September19袁2015were selected.The distance from the sexual gland to the radiation field in28cases of nasopharyngeal carcinoma radiotherapy was measured.The scattered radiation doses of the sexual gland place in the simulated image鄄guided verification and treatment condition was measured by the patrol dosimeter.Results The longest distances from the sexual gland to the radiation field was64cm and the shortest distance was51cm袁the mean value was渊55.29依0.53冤cm.The average received dose in the CBCT image guiding and verifying process was渊0.53依0.01冤mSv and its effective dose was渊0.11依0.01冤mSv;the average received dose in the radiotherapy process was渊74.57依1.01冤mSv and its effective dose was渊14.91依0.20冤mSv;the mean received dose during the whole treatment process was渊75.09依1.00冤mSv and its effective dose was渊15.02依0.20冤mSv.Conclusion The sexual gland receives the lower scattered radiation dose in male patients with nasopharyngeal carci鄄noma intensity鄄modulated radiotherapy.This sensitive organ during intensity鄄modulated radiotherapy should be fully assessed.
