MMP-2在乳腺癌中的表达及临床意义

摘　　要：目的:探讨明胶酶A(MMP-2)在乳腺正常组织、不典型增生、原位癌、浸润性癌组织中的表达水平,分析其与浸润性乳腺癌临床相关病理指标及术后生存情况的相关性。方法:收集正常乳腺组织20例、不典型增生28例、导管内癌40例、浸润性乳腺癌标本80例,用免疫组化SP法检测各组MMP-2的表达,分析其在浸润性乳腺癌中的表达水平与肿瘤大小、腋窝淋巴结受累情况、TNM分期、组织学分级、ER、PR、HER-2等临床病理指标之间的相关性;并分析其对乳腺癌患者术后生存状况的影响。结果:MMP-2在乳腺正常组织(阳性率20.0%)、乳腺不典型增生组织(阳性率21.4%)、原位癌组织(阳性率46.3%)、浸润性乳腺癌组织(阳性率71.3%)的表达依次增高;MMP-2阳性率在乳腺原位癌与不典型增生、浸润性癌与原位癌之间具有统计学差异(P<0.05)。在浸润性乳腺癌中,不同临床分期及淋巴结转移亚组MMP-2阳性率比较有统计学差异(P<0.05)。在原位癌中,MMP-2染色阳性组5年无病生存率(47.4%)低于阴性组(95.2%);在浸润性乳腺癌中,MMP-2染色阳性组5年无病生存率(56.9%)低于阴性组(86.5%),均有统计学差异(P<0.05)。结论:MMP-2参与了乳腺癌发生、发展的过程,并与乳腺癌预后相关,可作为预测乳腺癌预后的潜在肿瘤标志物。Objective:To study the expression of matrix metalloproteinase-2(MMP-2)in normal tissue of breast,atypical hyperplasia?ductal carcinoma in situ and invasive carcinoma?and analyze the associations with pathological features and clinical outcome in human breast cancer.Methods:Expressions of MMP-2in20cases of normal breast tissue,28cases of atypical hyperplasia,40cases of ductal carcinoma in situ(DCIS)and80cases of invasive carcinoma were determined by IHCCimmune ohistochemistry)technique respectively.The level of MMP-2expression in80cases of invasive breast cancer was analyzed against the clinical and pathology features such as tumor diameter,lymph node status and so on.All the120cases of breast carcinoma were followed-up.Results:The expression of MMP-2increased from normal tissue of breast(the positive rate20.0%)to atypical hyperplasia(the positive rate21.4%)and DCIS(the positive rate46.3%)and invasive breast cancer(the positive rate71.3%).There was statistical difference between DCIS and atypical hyperplasia(P<C〇.05).While there was statistical difference between invasive carcinoma and DCIS(P<C〇.05).The expression of MMP-2had statistically significant differences in some groups of different clinical stage and lymph node metastasis(P<C〇.05).Disease free survival rate of ductal carcinoma in situ group with MMP-2positive expression(47.4%)was lower than the negative group(95.2%)(P<C〇.05).Disease free survival rate of the invasive carcinoma group with MMP-2positive expression(56.9%)was lower than the negative group(86.5%)(P<C〇.05).Conclusions:MMP-2participate the course of breast cancer.It may be a prognostic indicator of breast cancer.
