摘　　要：目的回顾性分析原发性支气管肺类癌的CT征象,并结合病理分型,提高对该病诊断水平。方法收集我院经手术病理确诊的7例支气管肺类癌患者的临床及影像检查资料,分析病灶部位、与邻近支气管关系、形态、大小、密度、边界、动态增强后表现等,并结合文献复习。结果本组病例包括4例典型类癌,3例不典型类癌。中央型4例,周围型3例。病灶均为单发,类圆形、椭圆形5例,1例呈分支管状、1例呈不规则团块状伴晕征。其中2例中央型病灶主要位于支气管腔内并向腔外生长,形成"冰山征"、"平行征"。Objective To analyze the CT features of primary bronchial carcinoid correlated with the pathological type.Methods The clinical and imaging data of7patients with bronchial carcinoid were retrospectively analyzed.The lesion location,relationship of adjacent bronchi,morphology,maximum size,density,contour,and dynamic enhancement characteristics were recorded.Results The bronchial carcinoid was typical(4)or atypical(3),central(4)or peripheral(3),solitary round or oval nodule(5),tubular(1)or irregular mass(1).Two central lesions were protruding from the bronchial lumen displaying iceberg sign and tram-track signs.Average tumor diameter was29.4±2.3mm with1.71maximum and1.07minimum aspect ratios.The lesions showed homogeneous CT density(5/7,71.4%),calcification(1/7,14.3%)or halo(1/7,14.3%).The borders were slightly lobular(3/7,42.8%)or markedly lobular(1/7,14.3%).Marked contrast enhancement in the venous phase to54.1±11.4HU was seen in6/7lesions whereas1/7showed only slight enhancement.Mediastinal and hilar lymph node metastasis was observed in1patient.Conclusion Primary bronchial carcinoid should be considered when CT features of central endobronchial lesions with smooth margins,calcifications,and marked contrast enhancement.
