益气解毒中药联合肝动脉化疗栓塞术治疗原发性肝癌疗效及对T淋巴细胞亚群、GP-73、GPC-3水平的影响

摘　　要：目的观察益气解毒中药联合肝动脉化疗栓塞术治疗原发性肝癌疗效及对T淋巴细胞亚群、高尔基体糖蛋白-73(GP-73)、磷脂酰肌醇蛋白聚糖-3(GPC-3)水平的影响。方法将130例原发性肝癌患者随机分为2组,对照组65例给予肝动脉化疗栓塞术治疗,观察组65例在此基础上加用益气解毒中药治疗,观察2组治疗前后中医证候积分、肝功能指标水平、EORTCQLQ-C30评分、T淋巴细胞亚群水平、GP-73及GPC-3水平变化情况,统计2组近期疗效。结果 2组治疗后气短、神疲乏力、面色淡白、头晕眼花、舌淡及脉细积分均显著低于治疗前(P均<0.05),且观察组治疗后各项积分均显著低于对照组(P均<0.05);2组治疗后躯体功能、角色功能、情绪功能、认知功能及社会功能评分显著提高(P均<0.05),疲倦、恶心呕吐及疼痛评分显著降低(P均<0.05),且观察组治疗后各项指标改善情况均显著优于对照组(P均<0.05);对照组治疗前后T淋巴细胞亚群水平比较差异无统计学意义(P>0.05);观察组治疗后CD3+、CD4+及CD4+/CD8+水平均显著提高(P均<0.05),CD8+水平显著降低(P<0.05),且观察组治疗后各项指标改善情况均显著优于对照组(P均<0.05);观察组近期治疗总有效率显著高于对照组(P<0.05)。结论益气解毒中药联合肝动脉化疗栓塞术治疗原发性肝癌可有效缓解临床症状体征,保护肝功能,提高日常生活质量,调节机体免疫系统功能,降低GP-73和GPC-3水平。Objective It is to observe the therapeutic effect of Yiqi Jiedu traditional Chinese medicine plus transcatheter arterial chemoembolization(TACE)on hepatocellular carcinoma(HCC)and its influence on the levels of T lymphocyte subsets,Golgi glycoprotein73(GP73)and phosphatidylinositol proteoglycan3(GPC3).Methods130patients with primary liver cancer were randomly divided into2groups,65cases in the control group were treated with TACE,and65cases in the observation group were treated with Yiqi Jiedu traditional Chinese medicine on the basis of control group.The TCM syndrome score,liver function index,EORTCQLQ C30score,T lymphocyte subsets,GP73and GPC3levels were observed before and after treatment in the2groups,the short term effects of the two groups were compared.Results After treatment,the integral of shortness of breath,lassitude,pale complexion,dizziness,pale tongue,and pulse fine were significantly lower in the2groups(all P<0.05),and the observation group was significantly lower than the control group(P<0.05);after treatment,the scores of physical function,role function,emotional function,cognitive function,and social function were significantly improved but the scores of fatigue,nausea,vomiting and pain were significantly decreased in the2groups(all P<005),and the improvement of the indexes in the observation group was significantly better than that in the control group(P<005);there was no significant difference in the level of T lymphocyte subsets before and after treatment in the control group(P>0.05);after treatment,the levels of CD3+,CD4+and CD4+/CD8+were significantly increased but the level of CD8+were significantly decreased in the observation group(all P<0.05),and the improvement of the indexes in the observation group was significantly better than that in the control group(P<0.05);the total effective rate of the observation group was significantly higher than that of the control group(P<0.05).Conclusion Yiqi Jiedu traditional Chinese medicine combined with TACE can effectively relieve the symptoms an
