血小板与新生血管性青光眼的相关性研究

摘　　要：目的探讨血小板在糖尿病视网膜病变继发性新生血管性青光眼(NVG)中水平的变化及意义。方法采用回顾性,病例对照研究设计,收集2010年1月至2016年10月于泰安市中心医院就诊并确诊为NVG的患者88例,同时收集同时间段的糖尿病患者104例作为糖尿病对照组,及体检者96例作为正常对照组,并采集其年龄、性别、血小板参数、疾病史等资料。根据临床分期标准,将NVG患者分为开角型NVG及闭角型NVG亚组。采用独立样本t检验、方差分析及多因素Logistic回归分析对NVG与对照组间的血小板指标进行统计学分析。结果 3组间年龄和性别差异不具有统计学意义(P>0.05)。单因素方差分析显示:NVG患者血小板分布宽度(PDW)、血小板压积(PCT)、平均血小板体积(MPV)水平分别为(16.14±1.54)fl、(0.223±0.059)%和(10.69±1.53)fl,明显高于正常对照组的(13.69±2.99)fl、(0.196±0.043)%和(9.78±0.92)fl;NVG患者PDW水平显著高于糖尿病组[(13.89±3.35)fl];组间总体比较差异均有统计学意义(P<0.005)。开角型NVG及闭角型NVG亚组间血小板水平没有明显差异(P<0.05)。多因素Logistic回归分析,发现PDW(OR=1.519,95%CI:1.244~1.854,P<0.001)、MPV(OR=3.066,95%CI:1.156~8.134,P=0.024)为NVG发病的独立危险因素。结论本研究发现PDW、MPV与NVG存在相关性,提示血小板可能在NVG的发生中发挥重要作用。Objective To investigate the change of platelets in neovascular glaucoma secondary to diabetic retinopathy and analyze its clinical significance.Methods This retrospective,case-control study involved a total of288subjects enrolled from January2010to December2016at Taian Central Hospital,including patients with NVG secondary to diabetic retinopathy(n=88),diabetes mellitus without retinopathy(n=104),and healthy individuals(n=96).The data of all the subjects were collected.NVG was divided into two categories:open-angle NVG and close-angle NVG.Independent samples t-test,one-way ANOVA,and multiple logistic regression analysis were used for statistical analysis in this study.Results There was no statistical difference in mean age or gender among the three groups.The mean levels of platelet distribution width(PDW),plateletcrit(PCT),and mean platelet volume(MPV)were higher in the NVG group compared with the control group[(16.14±1.54)fl vs(13.69±2.99)fl,(0.223±0.059)%vs(0.196±0.043)%,(10.69±1.53)fl vs(9.78±0.92)fl,respectively].The mean level of PDW was higher in the NVG group compared with the diabetes mellitus group[(16.14±1.54)fl vs(13.89±3.35)fl].There was no significant difference between the open-angle NVG and close-angle NVG groups.Multiple logistic regression analysis revealed that PDW(OR=1.519,95%CI:1.244-1.854,P<0.001)and MPV(OR=3.066,95%CI:1.156-8.134,P=0.024)were independent risk factors for NVG.Conclusion PDW and MPV are associated with NVG,suggesting that platelets might play an important role in the development of NVG
