单孔与三孔全胸腔镜解剖性肺叶切除治疗早期肺癌的效果比较

摘　　要：目的比较单孔与三孔全胸腔镜解剖性肺叶切除治疗早期肺癌的效果。方法将120例早期原发性肺癌患者按手术方式不同分为试验组与对照组,每组60例。试验组行单孔全胸腔镜解剖性肺叶切除手术治疗,对照组行三孔全胸腔镜解剖性肺叶切除手术治疗,比较2组的手术时间、术中出血量、术后住院时间、清扫淋巴结枚数与站数、术后疼痛评分及术后并发症发生情况。结果 2组均顺利完成手术。2组手术时间、术中出血量、清扫淋巴结枚数与站数及术后并发症发生率比较差异均无统计学意义(P>0.05),试验组术后疼痛评分及术后住院时间均较对照组显著减少,差异有统计学意义(P<0.05)。结论单孔全胸腔镜手术应用于解剖性肺叶切除术是安全可行的;与三孔全胸腔镜手术比较,可减轻患者术后疼痛,缩短术后住院时间,提高患者近期的生活质量。ObjectiveTo compare the efficacies of uniportal and three-portal video-assisted thoracoscopic anatomic pulmonary lobectomy in early stage lung cancer.MethodsA total of120patients with primary early stage lung cancer were assigned to receive either uniportal(experimental group,n=60)or three-portal video-assisted thoracoscopic anatomic pulmonary lobectomy(control group,n=60).The operation time,intraoperative blood loss,hospital stay,postoperative pain score,incidence of complications and number of lymph nodes dissected were compared between the two groups.ResultsSurgeries were completed successfully in both groups.There were no statistical differences in operation time,intraoperative blood loss,number of lymph nodes dissected,incidence of complications between the two groups(P>0.05).Compared with control group,postoperative pain score and hospital stay were decreased in experimental group(P<0.05).ConclusionUniportal video-assisted thoracoscopic surgery is safe and feasible in anatomical lobectomy.Compared with three-portal video-assisted thoracoscopic lobectomy,it can reduce postoperative pain,shorten hospital stay and improve the quality of life in patients with early stage lung cancer.
