甲状腺乳头状癌BRAF V600E基因突变与临床病理特征相关性研究

摘　　要：目的探讨鼠类肉瘤滤过性毒菌致癌同源体B1(BRAF)V600E基因突变与甲状腺乳头状癌患者临床病理特征的相关性。方法回顾性分析110例确诊为甲状腺乳头状癌患者及38例甲状腺良性病变患者的临床病理资料,石蜡包埋组织经QIAGEN试剂盒提取DNA,采用实时荧光定量PCR(real-time PCR)法检测BRAF基因V600E点突变,分析BRAF V600E突变与临床病理特征的相关性。结果 93例甲状腺乳头状癌患者发现BRAF V600E基因突变(93/110),38例甲状腺良性病变患者BRAF V600E突变率为0%(0/38)。BRAF基因突变与甲状腺被膜外侵犯及临床高分期有关(P<0.05),而与性别、年龄、女性患者是否绝经、直径大小、多灶性发生、淋巴细胞性甲状腺炎、组织学亚型、包裹性或是浸润性、钙化、淋巴结转移情况无关(P>0.05)。结论 BRAF V600E基因突变可能与甲状腺乳头状癌侵袭性生物学行为相关,为预后判断提供依据。Objective To investigate the BRAF V600E mutation in papillary thyroid carcinoma and the relationship with clinicopathologic parameters.Methods A retrospective analysis was made on the clinicopathological data of110cases and38benign thyroid cases,the DNA was extracted from paraffin sections using the real-time PCR method to detect BRAF gene mutation.The correlation between the frequency of BRAF mutation and their clinicopathological characteristics were analyzed.Results BRAF V600E mutation was found in93cases(93/110),while no BRAF mutation was identified in38benign thyroid samples(0/38).There was significant correlation between BRAF gene mutations and extrathyroidal extension and advanced clinical stage(P<0.05),however there were no correlation with gender,age,menopause,tumor size,multicentricity,lymphocytic thyroiditis,histopathologic subtypes,encapsulate or infiltrative,calcification and lymph node metastasis(P>0.05).Conclusion BRAF V600E mutation has some correlation with the invasion ability of papillary thyroid carcinoma cells,which provides the basis for judging prognosis.
