载药微球与空白微球栓塞治疗原发性肝癌的对比分析研究

摘　　要：目的比较应用Calli Spheres可载药栓塞微球的药物缓释微球肝动脉化疗栓塞术(DEB-TACE)及应用8Spheres空白栓塞微球的肝动脉栓塞术(TAE)两种不同方法在原发性肝癌(HCC)治疗中的近期临床价值,并探讨化疗药物对HCC的治疗作用。方法回顾性收集分析2016年6月~2017年2月通过载药微球DEB-TACE及空白微球TAE两种不同方法初治的原发性肝癌患者共46例。其中载药微球组(Calli Spheres可载药栓塞微球+阿霉素)24例,空白微球组(8Spheres空白栓塞微球)22例。通过CT或MRI比较两组患者术后6个月肿瘤局部反应及肿瘤复发情况。同时比较两组患者术后并发症发生情况。结果两组患者基线临床特征无统计学差异(P>0.05)。术后6个月载药微球组肿瘤客观反应(OR)率显著高于空白微球组(χ~2=4.207,P=0.040)。同时,载药微球组肿瘤复发率显著低于空白微球组(χ~2=4.023,P=0.045)。两组患者术后并发症发生率无统计学差异(P>0.05)。结论与8Spheres空白栓塞微球TAE相比,Calli Spheres可载药栓塞微球DEB-TACE可显著提高肿瘤疗效,证实了化疗药物在HCC治疗中的重要作用,但远期临床疗效有待进一步随访研究。Objective To compare the short-term clinical value of drug-eluting beads transarterial chemoembolization(DEB-TACE)with CalliSpheres beads and transarterial embolization(TAE)with8Spheres beads in the treatment of hepatocellularcarcinoma(HCC).Methods The clinical records of46patients with HCC initially treated by DEB-TACE with doxorubicin-loadedCalliSpheres beads(24)and TAE with bland8Spheres beads(22)from June2016to February2017were retrospectively analyzed.CTor MRI was performed to evaluate the local tumor response and tumor recurrence at6months after treatment in both groups of pa鄄tients.Results There were no significant differences in patient characteristics between the two groups(P>0.05).The response rateand disease control rate in drug-loaded beads group were significantly higher(字2=4.207,P=0.040)whereas the tumor recurrencerate was significantly lower(字2=4.023,P=0.045)than those in bland beads group at6months after treatment.There was no signifi鄄cant difference in the incidence of complications between the two groups(P>0.05).Conclusion DEB-TACE with CalliSpheresbeads significantly improves the6-month curative effect of HCC compared to TAE with8Spheres beads.
