颌骨动静脉畸形CT影像特征与鉴别

摘　　要：目的分析颌骨动静脉畸形的CT表现特征,探讨CT检查对颌骨动静脉畸形的临床诊断价值。方法回顾性分析2016年8月~2017年6月我院8例首诊的颌骨动静脉畸形患者的CT影像资料及临床资料,就其位置、形态、边界、内部结构、邻近骨及软组织侵犯、增强后密度改变及周围血管情况等进行分析。结果本组病例中病灶发生于上颌骨2例(2/8,25%)、下颌骨6例(6/8,75%);7例(7/8,87.5%)颌骨呈局部溶骨性骨质吸收破坏,骨皮质变薄,骨髓腔间隙增大,骨小梁结构消失,其中1例颌骨内病灶可见明显纤细骨性分隔,1例(1/8,12.5%)表现为颌骨内囊样低密度灶,未见明显颌面部软组织肿块;8例(8/8,100%)增强后表现为颌骨内及颌面部软组织肿块呈明显强化,CT值约100~140 HU,其内可见增粗扭曲血管影;7例(1/8,87.5%)可见颈外动脉供血,粗大引流静脉引流入颈内静脉、颈外静脉,1例(1/8,12.5%)仅见病灶内强化小血管影。结论颌骨动静脉畸形具有典型的CT表现特征,CT增强检查可明确颌骨动静脉畸形的病灶范围及血管构筑,有助于提高对该疾病的诊断与认识,辅助临床进一步行介入栓塞治疗。Objective To analyze contrast enhancement features and diagnostic value of CT in mandibular arteriovenous malformation(AVM).MethodsThe clinical data and contrast-enhanced CT features of8patients with mandibular AVM including position,shape,border,internal structure,enhanced density change,and peripheral vasculature of the AVM as well as adjacent bone and soft tissue invasion were retrospective analyzed.Results CT in7/8(87.5%)AVMs showed destruction of the mandible with thinning of the bone cortex,increased marrow cavity and loss of trabeculae.One AVM showed a low-density lesion of the inner capsule of the maxilla without soft tissue mass.Contrast-enhanced CT showed maxillofacial soft tissue masses with blood vessel distortion and significant enhancement to CT values of100-140HU in all8AVMs.7/8AVMs were fed by the external carotid arteries with large veins draining into the internal and external jugular veins.Only small blood vessels were visualized in1lesion.Conclusion CT enhancement characteristics can delineate the extent and blood supply of maxillofacial AVM.
