一次性根管治疗慢性尖周炎的疗效观察

摘　　要：目的观察并分析一次性根管治疗与常规治疗对慢性尖周炎的临床效果。方法选取2016-02至2017-03长春市口腔医院收治的154例感染较轻或症状不明显的慢性尖周炎患者为研究对象,采用患者单盲随机法将其分成观察组(n=77)和对照组(n=77)。其中对照组采用常规治疗,而观察组给予一次性根管治疗,比较两组的治疗效果。结果治疗后,观察组疼痛缓解情况好于对照组,差异具有统计学意义(Z=-3.369,P=0.001);进一步比较显示:观察组中、重度疼痛发生率低于对照组,差异具有统计学意义(χ~2=14.659,P<0.001)。观察组治疗效果优于对照组,但差异无统计学意义(Z=-0.253,P=0.801);进一步比较,观察组治疗总有效率略高于对照组,但差异无统计学意义(P>0.05)。结论一次性根管治疗和常规治疗均对慢性尖周炎有良好的治疗效果。但一次性根管治疗能明显缩减治疗时间和就诊次数,减轻了患者的疼痛,从而增加了治疗依从性,是一种更便捷、有效的治疗方法,值得临床推广应用。Objective The objective of this study was to observe and analyze the clinical effect of one-session root canal therapy and routine treatment on chronic periapical periodontitis.Methods A sample of154patients with chronic periapical periodontitis treated in the Changchun Dental Hospital from February2016to March2017were selected and were divided into the observation group(n=77)and the control group(n=77)using the single blind semi random method.The control group was treated with routine treatment,while the observation group received one-session root canal therapy.The therapeutic effects of the two groups were compared.Results After treatment,the pain in the observation group was alleviated more effectively than the control group,and the difference was statistically significant(Z=-3.369,P=0.001);further comparison showed that the incidence rate of moderate and severe pain in the observation group was lower than the control group,and the difference was statistically significant(χ2=14.659,P<0.001).The therapeutic effect of the observation group was better than that of the control group,but the difference was not statistically significant(Z=-0.253,P=0.801).The total effective rate of the observation group was slightly higher than that of the control group,but the difference was not statistically significant(P>0.05).Conclusions One-session root canal therapy and routine treatment are effective to treat chronic periapical periodontitis.But one-session root canal therapy can significantly reduce the time taken to treat and the frequency of visits,relieve the pain of patients,and thereby increase the treatment compliance,which is a more convenient and effective therapy and is worthy of clinical application.
