RACK-1和Twist在宫颈癌组织中的表达及其临床意义

摘　　要：目的探讨RACK-1和Twist在宫颈癌中的表达及其临床意义。方法选取2006年5月-2010年6月在该医院手术治疗的宫颈癌患者70例,选取同期因宫颈良性病变行肿物剥除或附件切除的20例患者作为对照,采用免疫组织化学SP法检测70例宫颈癌组织(宫颈癌组)、70例宫颈癌癌旁组织(癌旁组)、20例正常宫颈组织(正常组)中Twist和RACK-1蛋白的表达,分析其与宫颈癌患者临床病理特征的关系。结果 (1)正常组、癌旁组、宫颈癌组的Twist阳性率分别为5.0%(1/20)、22.9%(16/70)和95.7%(67/70),差异有统计学意义(P<0.05);正常组、癌旁组、宫颈癌组的RACK-1阳性率分别为10.0%(2/20)、61.0%(43/70)和90.0%(63/70),差异有统计学意义(P<0.05)。(2)Twist表达与肿瘤分化程度、淋巴结转移及TNM分期有关(P<0.05)。RACK-1表达与肿瘤分化程度、浸润深度、淋巴结转移及TNM分期有关(P<0.05)。(3)Twist与RACK-1呈正相关(P<0.05)。(4)Twist高表达者和低表达者的5年总生存率分别为51.2%和29.2%[HR=11.637(95%CI:4.351,38.213)P=0.002];RACK-1高表达者和低表达者的5年总生存率分别为41.3%和35.3%[HR=10.143(95%CI:4.285,33.275)P=0.006]。结论 Twist和RACK-1的表达升高与宫颈癌侵袭性增强关系密切,其表达水平可判断宫颈癌患者的预后。Objective To study Twist and RACK-1expressions in cervical carcinoma,and analyze the clinical significance.Methods This study enrolled the cervical carcinoma tissues(cervical carcinoma group)and para-cancerous tissues(para-carcinoma group)of70patients with cervical carcinoma and the normal cervical tissues(control group)of20patients with benign cervical tumor from May2006to June2010in the Affiliated Hospital of Southwest Medical University.Twist and RACK-1expressions in the three groups were detected with immunohistochemical SP method.Their correlations with clinicopathological parameters of cervical carcinoma were analyzed.Results The positive expression rates of Twist in the control group,the para-carcinoma group and the cervical carcinoma group were5.0%(1/20),22.9%(16/70)and95.7%(67/70)respectively,with significant differences among them(P<0.05).The positive expression rates of RACK-1in the control,para-carcinoma and cervical carcinoma groups were10.0%(2/20),61.0%(43/70)and90.0%(63/70)respectively,with significant differences among them(P<0.05).The expression of Twist was correlated with TNM stage,degree of tumor differentiation and lymph node metastasis(P<0.05).The expression of RACK-1was correlated with TNM stage,degree of tumor differentiation,depth of invasion and lymph node metastasis(P<0.05).There was a positive correlation between Twist and RACK-1(P<0.05).The5-year overall survival rates for the patients with high Twist expression(+++)and low expression(+,++)were51.2%and29.2%[HR=11.637(95%CI:4.351,38.213),P=0.002].The5year overall survival rates of the patients with high RACK-1expression(+++)and low expression(+,++)were41.3%and35.3%[HR=10.143(95%CI:4.285,33.275),P=0.006].Conclusions The up-regulated expressions of Twist and RACK-1are closely related to the increased malignancy and invasion of cervical carcinomas.The expressions of Twist and RACK-1might be used as indicators for prediction of the prognosis of patients with cervical carcinomas.
