探讨妊娠期高血压疾病发病机制及防治的研究进展

摘　　要：目的:探讨妊娠期高血压疾病发病机制及防治措施。方法:选取120例娠期高血压疾病患者作为研究组,同时选取120例正常妊娠女性作为对照组。采用问卷调查、logistic回归分析、PCR及蛋白免疫印迹法分析并对比两组研究对象的基本资料。结果:年龄大于35岁的初产孕妇、孕前体重指数过高过低、多胎妊娠、糖尿病史、子痫前期史及肾脏疾病史是导致女性妊娠期高血压疾病的独立危险因素(P<0.05);研究组患者的AT1R基因及ACE基因水平、胎盘生长因子、活性氧水平、脂质过氧化物及Th1/Th2比值与对照组相比具有可比性(P<0.05)。结论:妊娠期高血压疾病发病机制与遗传学、免疫学及氧化应激有关,且年龄大于35岁的初产孕妇、孕前体重指数过高过低、多胎妊娠、糖尿病史、子痫前期史及肾脏疾病史是导致女性妊娠期高血压疾病的独立危险因素。Objective:To explore the pathogenesis and prevention measures of hypertensive disorder complicating pregnancy.Methods:120patients with gestational hypertension disease as the study group,and120cases of normal pregnant women as control group.Questionnaires,logistic regression analysis^PCR and Western blot were used to analyze and compare the basic data of the two groups.Results:The age of primiparous pregnant women,35year old pre pregnancy body mass index is too low^multiple pregnancy,history of diabetes^history of preeclampsia and kidney disease is the result of independent risk fac-tors of hypertensive disorders in pregnancy women(P<C0.05);placental AT1R gene and ACE gene level,the study group of patients with growth factors^the level of reactive oxygen species Thl,LPO and/Th2ratio compared with the control group comparable(P<C0.05).Conclusion:pregnancy induced hypertension pathogenesis and immunology and genetics^oxidative stress,and age more than35year old primiparous pregnant women,pre pregnancy body mass index is too low,multiple preg-nancy,history of diabetes,history of preeclampsia and kidney disease is the result of independent risk factors for pregnancy high blood pressure disease.
