摘  要:目的:探讨基于MIM软件的模板数据库中病例数对危及器官自动勾画的影响,指导临床选取最佳病例数构成自动勾画数据库。方法:选取150例鼻咽癌患者,按照数表法随机将其分为a组、b组、c组和d组,在MIM软件中建立a、b、c、d对应的四组患者的数据库,数据库包含CT图像和医生手动勾画的危及器官轮廓信息,另选取10例非数据库患者的头部CT图像进行危及器官的自动勾画,与医生手动勾画结果作为金标准进行比较。采用形状相似性系数(DICE)、豪斯多夫距离及Jaccard距离3个指标评价自动勾画的危及器官。结果:四组数据库自动勾画的危及器官DICE值在0.5~0.9之间,眼球、内耳、口腔、腮腺等器官勾画结果DICE值均在0.8以上。眼球、内耳、颞颌关节、晶体等危及器官或组织的豪斯多夫距离(标准差)均值<0.2。Jaccard距离值随模板数据库包含病例数增多而减小,其中内耳、颞颌关节、晶体和视神经均值<0.4。结论:模板数据库包含病例数越多,眼球、内耳、脊髓、视神经等器官DICE值越高,眼球、内耳、视神经等器官的豪斯多夫距离(标准差)值越小,各危及器官的Jaccard距离值越小。眼球、内耳、脊髓、视神经等危及器官宜选用病例数多的模板数据库进行自动勾画,模板数据库中包含的病例数对脑干、颞颌关节、晶体、口腔、腮腺等危及器官或组织的自动勾画无明显影响。Objective:To investigate the effect of the number of medical case for the automatic delineation on organ at risk based on template database of MIM software,and to guide clinical practice to select optimal amount of cases so as to constitute database of automatic delineation.Methods:150patients with nasopharyngeal carcinoma were divided into four groups that included of group a(10cases),group b(50cases),group c(100cases)and group d(150cases).And the database of four groups was constructed in the MIM software.And the database included of CT image and the outline information of organ at risk that delineated by manual operation of doctor.At the same time,the CT images of head of other10patients who weren't in the database were automatically delineated for organ at risk.And the automatic delineations were compared with gold standard that delineated by manual operation of doctor.The dice similarity coefficient(DICE),Hausdorff distance and Jaccard distance were used to evaluate the organ at risk of automatic outline.Results:The DICE values of four group were between0.5and0.9.And all of the DICE values of eyeball,inner ear,oral cavity,parotid gland and other organs were higher than0.8.And all of the means of Hausdorff distance(standard deviation)of eyeball,inner ear,temporomandibular joint,crystal and other organ or tissue at risk were less than0.2.The Jaccard distance was minishing with the increasing of amount of cases in the template database,and all of the means of inner ear,temporomandibular joint,crystal and optic nerve were less than0.4.Conclusion:With the increasing of amount of cases in the template database,the DICE values of eyeball,inner ear,spinal marrow,optic nerve and other organs will be higher,while the Hausdorff distances(standard deviation)of eyeball,inner ear,optic nerve and other organs will be smaller,and the Jaccard distances of each organ at risk will also be smaller.The organs at risk included of eyeball,inner ear,spinal marrow,optic nerve and others should adopted template database with more amo
