摘　　要：目的调查重症监护室(ICU)使用身体约束患者的相关特征及影响因素。方法采用分层随机抽样法抽取重庆市3家三级甲等综合医院的98例ICU住院患者,使用《ICU患者身体约束评估量表》进行横断面调查。结果98例患者中有38例患者使用身体约束,身体约束使用率为38.8%,其中34例约束部位为双上肢腕部,4例约束部位为四肢。患者的主要临床诊断为脑出血、呼吸衰竭、胰腺炎、颅脑损伤、骨折和CPR复苏术后。logistic回归分析结果显示,患者意识清醒、定向力正常、昏迷程度及合作程度为使用身体约束的保护因素。结论 (ICU)患者身体约束使用率高,身体约束的使用主要受患者行为等级影响,应根据我国实际情况制定ICU住院患者身体约束使用规范及标准,加强对临床护士的培训,指导临床对身体约束的合理使用。Objective To investigate the body constraint related characteristics and influencing factors for patients in intensive care unit.Methods A cross sectional study was conducted with《ICU physical restraint assessment scale》among98patients in ICU of three grade three A general hospitals of Chongqing.patients were selected by stratified random sampling method.Results Of the98patients,38had physical constraints,and the use of physical constraints was38.8%.The restricted parts of the34people were the wrist of the double upper limbs and the restricted parts of the4people were extremities.The main clinical diagnosis of the patients was cerebral hemorrhage,respiratory failure,pancreatitis,craniocerebral injury,fracture and CPR resuscitation.The results of logistic regression analysis showed that conscious consciousness,normal orientation,degree of coma and degree of cooperation were the protective factors for the use of physical constraints.Conclusion Patients in ICU have a high rate of use of physical constraints,and the use of physical constraints is mainly affected by the patient's behavior grade.According to the actual situation of China,we should draw up the norms and standards for the physical restraint of ICU inpatients,strengthen the training of clinical nurses,and guide the rational use of clinical physical restraint.
