低压子宫内膜吸取取样技术在子宫内膜病变中的早期诊断价值分析

摘　　要：目的探讨低压子宫内膜吸取取样技术在子宫内膜病变中的早期诊断价值。方法收集2016年1月~2017年1月就诊于本院有指征而需行子宫内膜评估的患者300例,先后采用低压子宫内膜吸引取样技术与诊断性刮宫术采集子宫内膜组织,并进行组织学诊断。比较两组诊断结果、手术时间、术中出血量、术后出血停止时间、疼痛视觉模拟(VAS)评分、患者检查满意度及样本质量。结果低压子宫内膜吸取取样与刮宫取样诊断结果基本一致率达96.33%(289/300);与刮宫取样病理诊断结果相对照,低压子宫内膜吸取取样对于病理性子宫内膜病变诊断的灵敏度为54.17%,特异度为100.00%,阳性预测值100.00%,阴性预测值96.17%,约登指数为0.54。低压子宫内膜吸取取样较刮宫取样手术时间明显缩短,术中出血量明显减少,VAS评分明显降低,满意度评分明显升高,差异有统计学意义(P<0.05)。低压子宫内膜吸取取样样本满意率为91.67%,刮宫术样本满意率为94.67%,两组比较差异无统计学意义(P>0.05)。结论低压子宫内膜吸取取样技术是一种有效、微创、操作简便的子宫内膜病变检查取材手段。Objective To analyze the early diagnostic value of low-pressure endometrial aspiration sampling technique in endometrial lesions.Methods From January2016to January2017,300patients with indications and in need of endometrial assessment who were admitted to our hospital were collected.The patients were successively given low-pressure endometrial aspiration sampling technique and diagnostic curettage for the collection of endometrial tissues,and histological diagnosis was carried out.The diagnostic results,surgery time,intraoperative blood loss,postoperative bleeding stop time,pain visual analog score(VAS),patients'satisfaction with the examinations and sample quality were compared between the two groups.Results The basic coincidence rate of low-pressure endometrial aspiration sampling and curettage sampling was96.33%(289/300);compared with the results of pathological diagnosis of curettage sampling,the sensitivity of low-pressure endometrial aspiration sampling in the diagnosis of pathological endometrial lesions was54.17%,specificity was100.00%,positive predictive value was100.00%,negative predictive value was96.17%,and Youden index was0.54.The surgical time of low-pressure endometrial aspiration sampling was shorter than that in curettage sampling,the intraoperative blood loss was significantly reduced,VAS score was significantly decreased,and the score of satisfaction was significantly increased.The differences were statistically significant(P<0.05).The satisfaction rate of low-pressure endometrial aspiration sampling was91.67%,and the satisfaction rate of curettage was94.67%.There was no statistically significant difference between the two groups(P>0.05).Conclusion Low-pressure endometrial aspiration sampling technique is an effective,minimally invasive and easily operable means of examination and sampling for endometrial lesions.
