猪苓多糖注射液联合顺铂腹腔给药治疗胃癌腹腔积液患者的疗效观察

摘　　要：目的观察猪苓多糖注射液联合顺铂腹腔给药治疗胃癌腹腔积液的临床疗效,并对其机制进行分析。方法 66例胃癌伴恶性腹水患者随机分为治疗组和对照组,每组33例。治疗组应用猪苓多糖注射液联合顺铂腹腔给药;对照组给予顺铂腹腔给药。治疗1个月后对2组的治疗效果及不良反应进行比较。ELISA法检测用药前后外周血白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平;实时定量PCR检测用药前后腹水中Gil1、基质金属蛋白酶-2(MMP-2)、MMP-9水平。结果治疗组患者腹水量、生活质量评分改善程度均比对照组明显,且治疗组不良反应发生率低于对照组(P<0.05)。2组治疗前外周血IL-6、TNF-α水平差异无统计学意义(P>0.05);治疗后2组IL-6、TNF-α水平均明显低于术前(P<0.05),治疗组IL-6和TNF-α变化程度优于对照组(P<0.05)。治疗前2组腹水Gil1、MMP-2、MMP-9的m RNA表达差异无统计学意义(P>0.05);治疗后2组Gil1、MMP-2、MMP-9的m RNA水平均明显低于术前(P<0.05),治疗组Gil1、MMP-2和MMP-9的m RNA变化程度优于对照组(P<0.05)。结论猪苓多糖注射液联合顺铂腹腔给药对胃癌恶性腹腔积液有良好的效果,其机制可能与抑制炎性因子,降低Gil1、MMPs的表达。Objective To observe the therapeutic effects of polyporus polysaccharide injection combined with cisplatin intraperitoneal injection on seroperitoneum due to gastric cancer,and to explore their action mechanism.Methods Sixty-six patients with gastric cancer complicated by malignant ascites were randomly divided into observation group and control group,with 33 patients in each group.The patients in observation group were treated by polyporus polysaccharide injection combined with cisplatin intraperitoneal injection,however,the patients in control group were treated by cisplatin intraperitoneal injection only.After 1-month treatment,the clinical curative effects and adverse reactions were evaluated,moreover,the serum levels of IL-6,TNF-αwere detected by ELISA,and the levels of Gil1,MMP-2 and MMP-9 mRNA were detected by real time PCR.Results After treatment,the ascites volume improvement degree and scores of life quality in observation group were superior to those in control group,moreover,the incidence rates of adverse reactions in observation group were significantly lower than those in control group(P<0.05).Before treatment there were no significant differences in the serum levels of IL-6,TNF-αbetween two groups(P>0.05).After treatment the serum levels of IL-6,TNF-αwere significantly decreased in both groups(P<0.05),moreover,the changes of serum levels of IL-6 and TNF-αin observation group were superior to those in control group(P<0.05).Before treatment there were no significant differences in the expression levels of Gil1,MMP-2,MMP-9 mRNA in ascites between two groups(P>0.05).After treatment the expression levels of Gil1,MMP-2,MMP-9 mRNA significantly decreased in both groups(P<0.05),moreover,the changes of Gil1,MMP-2 and MMP-9 mRNA in observation group were superior to those in control group(P<0.05).Conclusion The polyporus polysaccharide injection combined with cisplatin intraperitoneal injection has good therapeutic effects on seroperitoneum due to gastric cancer,and its action mechanism may be correlated
