摘　　要：目的 探讨早期心理干预辅助化学治疗对非小细胞肺癌（non-small cell lungcancer，NSCLC）患者抑郁状态及血清血管内皮生长因子（VEGF）的影响；方法 观察47例初诊NSCLC患者的抑郁状态，并随机分为化学治疗组（n=23）及早期心理干预组(n=24)，分别评价化疗前后患者抑郁状态，采用临床精神卫生症状自评量表（SCL-90）评估NSCLC缓和心理特征，采用酶联免疫吸附法（ELSIA）测定血清VEGF水平。结果共有29例患者（61.7%）诊断肿瘤后抑郁，其中化学治疗组15例，心理干预组14例，2组卒中后抑郁发生率比较，差异无统计学意义（p>0.05）；心理干预组患者SCL-90评分较常规治疗组患者显著降低，心理干预组患者各血清炎性因子水平均低于常规治疗组，2组差异均具有统计学意义。结论早期心理干预能够通过降低急性脑梗死患者血清炎性因子水平而减少卒中后抑郁的发生，并能有效改善卒中后患者的心理障碍。Objective To investigate the relationship between early psychological intervention and serum inflammatory factors in patients with non-small cell lung cancer(NSCLC).Methods178cases with acute cerebral infarct were randomly divided into conventional therapy group(n=88)and early psychological intervention group(n=90).The morbidity of PSD were assessed,and self-depression scale(SDS)and Hamilton Depression Scale(HAMD)were measured at14days after admission.Self-reporting Inventory(SCL-90)was used to evaluate the psycholoigcal features in patients with acute cerebral infarct.Results There were65cases diagnosed as PSD among acute cerebral infarct.Among them,the prevalence of PSD in conventional therapy group(47.7%)was signif icantly increased than that of psychological intervention group(25.5%)(X2=9.44,p<0.01).The score of SCL-90in the early psychological intervention group was decreased significantly than in the conventional therapy group;The serum hs-CRP,IL-2.IL-6and INF-αwere decreased signifcantly compared with the conventional therapy group.Conclusion The early conventional intervention could diminished the prevalence of PSD in patients with acute infarct according to decreasing the serum level of inflammatory factors.Our study showed that early psychological intervention could improve the psychological disorders in patients with PSD.
